摘　　要：美国电子病历发展经历了自由发展、政府宏观推动、依托经济手段推动三个阶段。其中,研究、标准化和政府三个因素互相促进,起到关键作用。美国电子病历应用在取得显著进展的同时还存在不少障碍。当前,美国政府通过建立标准、产品认证、奖励应用三个环节,大力推动电子病历的有效使用。美国电子病历发展过程带给我们的启示是:电子病历的发展是一个长期复杂的过程,推进过程需要清晰具体的行动目标和激励措施,需要系统筹划协调发展,需要建立和理顺投入回报机制。The development process of EHR in U.S. hospital has experienced three stages in which driving forces are respectively from the private sector, the government and the economic incentive. During this course, three actors of research facilities, standardization facilities and the government enhanced each other and played key roles to push the EHR forward. Although the use of EHR in U.S. hospital had been obtained remarkable progress, there were stilt some obstacles ahead. Currently, the U.S. government takes the new steps to prompt the EHR adoption and push the EHR toward meaningful use. The approach is three steps： set up standards, certify products and provide incentives for use. From the development process, we could draw some inspiration. Firstly, EHR development and adoption is a long and complicated process. Secondly, the government EHR promotion plan should have clear and specific goals and incentives. Thirdly, the promotion effort needs overall planning and multiparty coordinating. Lastly, a ROI mechanism is important for EHR adoption.
