翻转伸肌腱瓣重建中央束修复纽扣畸形

摘　　要：目的介绍应用翻转伸肌腱瓣重建中央束修复纽扣畸形的方法和疗效。方法对l5例纽扣畸形者,在手指近、中节背侧作"z"形延长切口,在近侧断端的中央腱上做一翻转的肌腱瓣,翻转绕远侧指间关节处的伸肌腱下通过,折返回来相互重叠缝合。术后近侧指间关节背伸5°克氏针斜行贯穿固定。3周后去除内固定进行功能锻炼。结果术后随访4个月～2a,平均14个月。采用Caroli功能评定法:优l1例,良3例,优良率达93.3%。结论采用翻转伸肌腱瓣重建中央束修复纽扣畸形操作简单,疗效可靠。Objective To report our usage of flip extensor tendon sheet which is construct the center beam tendon to rpair the buttonhole deformity, and introduce the method and outcome of this way. Methods Fifteen patients with the buttonhole deformity, were treat by ＂z＂ shaped extension incision from the dorsal of the finger near and section, then to do a flip tendon sheet from the proximal breakside of the central tendonon to overlap under through the distal interphalangeal,the proximal interphalangeal was fixed with oblique Kirschner dorsiflexion about 58in the postoperative,to do functional exercise after remove the fixation about 3 weeks later. Results The following time ranged from 4 months to 2 years, an average of 14 months. Caroli functional assessment method used： excellent 11 cases, good in 3 cases, good rate of 93.3%. Conclusion Using flip extensor tendon sheet for reconstructing the central beam tendon to repair the buttonhole deformity is simple operation, the effection is reliable.
