摘 要:目的 探讨"双线定位法"在颈椎间盘CT检查中的临床应用价值.方法 征求患者同意后对500例临床拟诊为颈椎退行性病变的患者进行颈椎容积扫描,得到容积扫描数据后在EBW (Extended Brilliance Workspace)4.0工作站对C3~4、C4~5、C5~6、C6~7椎间盘进行模拟常规椎间盘扫描横轴位重建及"双线定位法"颈椎间盘横轴位重建.分别观测比较2种重建模式对正常及异常椎间盘(膨出、突出及脱出)的检出能力,并进行χ2检验来验证差异是否具有统计学意义(P〈0.05有 统计学意义).结果 共2 000个椎间盘中,常规扫描和"双线定位法"分别检出异常椎间盘1 389(69.4%)、1 562(78.1%)个,差异具有统计学意义(P值为0.000).C3~4、C4~5、C5~6、C6~7椎间盘异常检出例数中,常规扫描分别为231(46.2%)、326(65.8%)、439(87.8%),393(78.6%);"双线定位法"颈椎间盘横轴位重建分别为262(52.4%)、351(71.2%)、498(99.6%),451(91.2%);后者较前者检出率分别提高了6.2%、5.4%、11.8%,12.6%,除C4~5椎间盘检测结果外,差异均具有统计学意义(P值分别为0.029、0.052、0.000、0.000).结论 "双线定位法"颈椎间盘横轴位重建能够更加准确地显示颈椎间盘的状态,为临床明确颈椎退行性病变患者的诊断提供更多有效的影像资料.Objective To evaluate the clinicaI applied value of "double lines orientation method" in cervical disc at CT examination. Methods 500 patients of clinical suspicion to be diagnosed with cervical vertebral degenerative disease was seeked informed consent and then underwent CT volumetric scanning. After volumetric scanning data obtained, reconstruction of imitation routine intervertebral discs examination and "double lines orientation method" intervertebral discs reconstruction were undertaken in Extended Brilliance Workspace (EBW) 4.0 workstation. The signs of normal or abnormal(swelling, bulging, and extrusion) intervertebral discs were observed with two scan modes respectively. The detecting capability of disc diseses was evaluated comparatively between two different scanning methods using x^2 test, P〈0.05 was statistically significant. Results 1389 ( 69.4 % ) and 1562 ( 78.1% ) disordered discs were detected by routine scans and "double lines orientation method" respectively in all 2 000 cervical intervertebral discs, there was statistical significance between them(P=0. 000). For C3-4,, C4-5, C5-6, C6-7 discs , 231(46.2%) ,326(65.8%) , 439 (87.8%) and 393(78.6%) ,262(52.4%),351(71.2%) ,498(99.6%) and 451(91.2%) disordered discs were detected with routine scans and "double lines orientation method" respectively, the latter than the former detecting rates increased by 6.2 %,5.4 %, 11.8 and 12.6 %, respectively , the differences were statistical significances except C4-5 (P= 0. 029 , 0. 052 , 0. 000 and 0. 000 , respectively). Conclusion "Double lines orientation method" cervical disc axial reconstruction can show the status of cervical disc more accurately, and can provide more image data for patients with degeneration of cervical vertebra to make effective clear diagnosis .
