摘  要:目的总结我院2007年以来应用大剂量地塞米松(Dex)治疗慢性特发性血小板减少性紫癜(ITP)的疗效和不良反应及与加用静脉丙种球蛋白治疗ITP患者的疗效对比。方法将慢性ITP患者21例分为A、B、C三组。A组12例采用大剂量Dex治疗,B组8例采用大剂量Dex联合静脉丙种球蛋白输注治疗,C组1例采用大剂量Dex联合静脉丙种球蛋白输注及促血小板生成素治疗,比较各种治疗方案疗效及不良反应。结果 A组总反应率为75.00%,B组总反应率为62.50%,两组比较差异无统计学意义(P>0.05);A组随访病例12例,长期反应率33.33%,B组随访病例8例,长期反应率25.00%,两组比较差异无统计学意义(P>0.05)。结论大剂量Dex治疗ITP效果好,维持时间长,不良反应小,患者依从性好,而联合静脉丙种球蛋白治疗并未增强其疗效。Objective To summarize the effect and adverse reaction of high-dose dexamethasone(Dex) treating patients with chronic idiopathic thrombocytopenic purpura(ITP) and contrast with ITP patients administrating intravenous immunoglobulin since 2007 in Altay region people's hospital. Methods All of 21 patients with chronic ITP were divided into A,B,C group. A group included 12 cases ITP patients with large doses Dex treatment,B group contained 8 cases ITP patients administrating large doses Dex and intravenous immunoglobulin, 1 case patient consisted of C group,which administrated large doses Dex combining intravenous immunoglobulin and thrombopoietin.To compare with the effect and adverse reaction between group A and B. Results The total response rate of group A and B was 75.00%,62.50% respectively. There was no statistically significant between group A and B(P〉0.05).Group A included follow-up 12 cases and long-term response rate was 33.33% ,long-term follow-up 8 cases consisted of group B and response rate was 25.00%.There was no statistically significant between the two groups too (P〉0.05). Conclusion The patients administrating high-dose Dex has good effect and long maintenance,small adverse reaction and good compliance,and patients with intravenous immunoglobulin have not enhance its curative effect.
