清热解毒化痰散结方对聚合性痤疮患者雄激素受体的影响

摘　　要：目的观察清热解毒化痰散结方对聚合性痤疮患者皮损内白细胞雄激素受体(AR)的影响。方法选取临床诊断为聚合性痤疮并符合中医痰热互结证型的患者126例,采用不平衡指数法结合患者就诊顺序分为治疗组42例,予清热解毒化痰散结方内服配合外用自拟痤疮洗剂(金银花等);对照1组42例,予内服美满霉素治疗配合外用自拟痤疮洗剂;对照2组42例,单纯外用自拟痤疮洗剂。用免疫组化方法检测每组患者治疗前后皮损处的白细胞雄激素受体。结果治疗组治疗后的AR阳性率低于治疗前(P<0.05),且治疗组的AR阳性率与对照组1组、对照2组相比,差异均有统计学意义(P<0.05)。结论清热解毒化痰散结方降低聚合型痤疮患者的AR明显优于西药组及单纯外治组,为清热解毒散结法治疗痤疮提供了依据。Objective To observe the impacts of the formula for clearing heat,detoxification,resolving phlegm and removing masses on leukocyte androgen receptor in skin lesions for the patients with acne conglobata.Methods 126 cases of acne conglobata were diagnosed in clinic and in compliance with the interaction of phlegm and heat pattern in Chinese medicine.According to imbalance index method and visiting sequence,they were divided into a treatment group（42 cases）,treated with the formula for clearing heat,detoxification,resolving phlegm and removing masses for internal administration and the self-prepared acne lotion（Lonicera Japonica）for external application;No.1 control group（42 cases）,treated with minocycline for internal administration and the self-prepared acne lotion for external application and No.2 control group（42 cases）,treated simply with the self-prepared acne lotion for external application.Immunochemical assay was adopted to detect the level of leukocyte androgen receptor in skin lesions before and after treatment in each group.Results The positive rate of androgen rate after treatment was lower than that before treatment in treatment group（P0.05）and the statistical significances presented as compared with No.1 control group and No.2 control group（P0.05）.Conclusion The formula for clearing heat,detoxification,resolving phlegm and removing masses reduces androgen receptor for the patients with acne conglobata,which is superior apparently to western medicine group and simple external application group.This research provides the evidence for the treatment of acne in terms of clearing heat,detoxification,resolving phlegm and removing masses.
