右美托咪定在ICU患者镇静中的疗效分析

摘　　要：目的评估右美托咪定在ICU患者镇静中的效果及安全性。方法符合人选标准的ICU患者60例随机分为右美托咪定组30例和咪达唑仑组30例。观察两组患者在目标镇静范围内镇静时间的百分比、谵妄发生率、停药后唤醒时间、机械通气时间和ICU留住时间、血压和心率不良反应事件等指标。结果两组达到目标镇静范围内时间的百分比相似；右美托咪定组谵妄发生率明显低于咪达唑仑组，右美托咪定组械通气时间和停药后唤醒所需时间短于咪达唑仑组，差异有统计学意义；两组患者ICU留住时间相似；右美托咪定组心动过缓的发生率更高，咪达唑仑组心动过速发生率更高；两组低血压发生率和高血压的发生率相似。结论右美托咪定使ICU患者达到目标镇静的同时，唤醒时间和机械通气的时间缩短，谵妄发生率降低，可作为ICU镇静的推荐药物。Objective To investigate the effect and reliability of dexmedetomidine in sedation on patients in intensive care unit. Methods 60 patients who met the inclusion criteria were randomly divided into dexmedetomidine group （ n = 30） and midazolam group （ n = 30）. Observe the percentage of sedative time in target sedative scope, the incidence of delirium, the awakening time, the length of mechanical ventilation , the length of ICU, the adverse reaction of blood pressure and heart rate. Results There was no significant difference in the percentage of sedative time in target sedative scope between two groups. The incidence of delirium of dexmedetomidine group was lower than that of midazolam group. The awakening time and the length of mechanical ventilation of dexmedetomidine group were shorter than that of midazolam group. There were statistical significance between two groups. The length of ICU of two groups was similar. The incidence of bradycardia in dexmedetomidine group was higher while the incidence of tachycardia in midazolam group was higher. There was no significant difference in the incidence of hypotension and hypertension between two groups. Conclusion Dexmedetomidine could make patients achieve the target sedtion, while shortening the awakening time and length of mechanical ventilation and decreasing the incidence of delirium. It could be the recommended drug for sedation in ICU.
