体重、身高及体重指数与双能X线骨密度仪和定量CT测量腰椎骨密度的关系

摘　　要：目的研究体重、身高及体重指数对双能X线骨密度仪(DXA)和定量CT所测腰椎骨密度的影响。资料和方法采用DXA及QCT分别测量各种原因来北京积水潭医院进行腰椎骨密度检查的患者。其中DXA检查患者12092例,年龄12～95岁,女性9788例,男性2304例,平均年龄58.9岁。QCT检查患者495例,年龄18～88,女性310例,男性184例,平均年龄为62.0岁。DXA和QCT测量L2～4椎体的平均骨密度。体重、身高及体重指数与骨密度的关系采用相关分析。结果 DXA骨密度与体重的相关性最大,r=0.479,决定系数r2=0.229。DXA骨密度与身高及体重指数的相关性分别为r=0.289,r=0.235;对应决定系数r2=0.084,r2=0.055。QCT骨密度与身高及体重都为弱相关,r=0.155和r=0.129;决定系数分别为r2=0.024,r2=0.017。QCT骨密度与体重指数相关性无统计学意义,r=0.031。结论体重与DXA与QCT所测得骨密度都存在明显相关,但QCT相关性较弱;QCT骨密度测量结果受身高、体重影响小。Purpose To evaluate the influence of weight, height and body mass index （BMI） on bone mineral density （BMD） measurements using quantitative computed tomography （QCT） and dual X-ray absorptiometry （DXA）. Material and Methods BMD values of the spine were obtained using DXA and QCT. The DXA BMD measurements were performed in a cohort of 12 092 patients （2 304 men and 9 788 women, mean age 58.9 years, range 19-95 years）. The QCT BMD measurements were performed in a cohort of 494 patients （184 men and 310 women, mean age 62.0 years, range 18-88 years）. Mean BMD of the lumbar spine （L2-4） was calculated. Correlation analysis was used to determine the relationship between BMD and anthropometric parameters including height, weight and BMI. Results The correlation of weight, height and BMI with DXA BMD was statistically significant （r=0.479, r=0.289, r=0.235 respectively）. The correlation of height and weight with QCT BMD has statistically significant differences （r=0.155, r= 0.129）, when BMI was not significantly correlated with QCT BMD. Conclusions The correlation of body weight with DXA BMD is stronger than that with QCT BMD. QCT BMD is less related to body weight and height.
