摘  要:目的:探讨内蒙古高氟地区血清维生素D含量和2型糖尿病的关系,为临床合理防治2型糖尿病提供依据。方法:本研究均选择本地居住10年以上居民,其中2型糖尿病75例,对患者的血清25羟基维生素D[25(OH)D3]含量、血清钙磷、血糖(BG)、糖化血红蛋白(HbA1c)、胰岛素、C肽、血脂谱、体重指数(BMI)、胰岛素抵抗指数和β细胞功能指数(HOMAβc)等指标进行综合分析,同时选取51例健康人的血清25(OH)D3含量作为对照。结果:2型糖尿病组与健康对照组的25(OH)D3水平比较,差异有高度统计学意义(P<0.01),分别为(2.87±2.83)、(14.38±14.01)nmol/L,两者均明显低于其他地区和国家最低标准(50 nmol/L);血清25(OH)D3水平和HbA1c、BMI和空腹血糖(FBG)呈负相关(r=-0.240、-0.231、-0.392,均P<0.05);血清25(OH)D3水平和HOMAβc呈正相关(r=0.274,P<0.05)。结论:内蒙古高氟地区维生素D水平较其他地区和国家明显降低;维生素D水平和2型糖尿病的发病和病情严重程度有关。给予本地区居民补充维生素D,一方面可以减少高氟损害,另一方面可以延缓2型糖尿病的病情。Objective: To determine the relationship between vitamin D levels and type 2 diabetes mellitus in the endemic fluorosis area of Inner Mongolia Autonomous Region,and to provide evidence of reasonable prevention and treatment method to type 2 diabetes patients in clinic.Methods: All of the groups had lived in the local region above ten years.Serum 25-hydroxyvitamin D,calcium and phosphorus,fasting blood glucose,HbA1c,body mass index(BMI),serum lipid profile,fasting blood insulin,insulin resistant index(HOMAIR) and β-cell function(HOMAβ-cell) were analyzed in 75 patients with type 2 diabetes,and serum 25-hydroxyvitamin D was detected in 51 healthy adults who were assessed as control group.Results: Mean(SD) 25-hydroxyvitamin D levels of type 2 diabetes group and health control group were(2.87±2.83) nmol/L and(14.38±14.01) nmol/L respectively,and there was a significant difference between them(P0.01),which noticeably decreased than lowest normal of the other areas and countries(50 nmol/L).Negative correlation were observed between serum 25-hydroxyvitamin D levels and glycated hemoglobin,body mass index and fasting glucose(r=-0.240,-0.231,-0.392,all P0.05),and a positive correlation was observed between serum 25-hydroxyvitamin D levels and HOMAβc(r=0.274,P0.05).Conclusion: Vitamin D levels of type 2 diabetes mellitus in the endemic fluorosis area of Inner Mongolia noticeably decreased than lowest normal of the other areas and countries.There is a negative correlation between serum 25-hydroxyvitamin D levels and type 2 diabetes mellitus.Supplementation of vitamin D in the local region peoples will be a better way of precaution and therapy of type 2 diabetes and relieve fluorosis meanwhile.
