经支气管镜氩离子凝固术治疗气管狭窄的效果及护理

摘　　要：目的：分析经纤维支气管镜（纤支镜）氩等离子体凝固（argen plasma coagulation,APC）治疗在气道狭窄中的应用价值,总结喉气管狭窄患者外科治疗和护理的经验。方法：回顾性分析2009年2月~2011年2月就诊于我院耳鼻咽喉头颈外科的15例喉气管狭窄患者的临床资料。主要应用ERBE 300型APC治疗器经纤支镜对患者进行治疗。结果：本组15例患者采用经纤维支气管镜（纤支镜）氩等离子体凝固治疗成功。15例患者进行了22次APC治疗（10例1次,3例2次,2例3次）,显效及部分有效12例（80.00%）,轻度有效2例（13.33%）,无效1例。气促评分：15例患者术前气促评分为（1.9±0.4）分,治疗后为（0.8±0.5）分,差异有统计学意义（t=7.386,P〈0.05）。结论：经纤支镜APC治疗气道狭窄是安全有效的,术前进行心理护理、完善各项相关检查、合理氧疗、全身营养、充分麻醉以及良好的术后护理可保证预期的治疗效果。Objective： To analyze the value of the treatment of fiberoptic bronchoscope（bronchoscopy） argon plasma coagulation（argen plasma coagulation,APC） in the airway stenosis,summarize surgical treatment and care experience of patients with laryngeal and tracheal stenosis.Methods： Clinical data of 15 patients of laryngeal stenosis in our hospital from February 2009 to February 2011 were analyzed retrospectively.Patients were treated by applied ERBE 300-type APC with bronchoscopy.Results： APC treatments by bronchoscopy of 15 patients were all success.15 patients were given APC treatment for 22 times（10 cases of 1 time,3 cases of 2 times,2 cases of 3 times）,and some markedly effective of 12 cases（80.00%）,mildly effective of 2 cases（13.33%）,ineffective of 1 case.The score of shortness of breath score of 15 patients were（1.9 ± 0.4） scores before treatment,and（0.8±0.5） scores after treatment,the difference was significant（t=7.386,P0.05）.Conclusion： The APC by bronchoscopy in the treatment of tracheal stenosis is safe and effective,preoperative psychological care,the relevant inspection,rational oxygen therapy,general nutrition,adequate anesthesia and good postoperative care need to take in order to ensure the desired therapeutic effect.
