全喉切除根治性颈廓清术后并发症的观察及护理干预

摘　　要：目的总结全喉切除根治性颈廓清术在治疗喉癌中的临床效果及术后主要并发症的观察和护理。方法回顾性分析我科2000年1月～2006年1月收治的81例喉癌患者,按UICC喉癌TNM分区[1],T1N0M026例,T2N0M025例,T2N1M017例,T3N2M013例,且符合全喉切除加根治性颈廓清术手术适应症。结果本组81例患者术后无1例死亡,均临床治愈出院,随访5年,死亡13例,其中4例因颈部转移拒绝再手术死亡,2例因肺转移死亡,2例因肺部感染死亡,3例死于脑溢血,2例死因不详,3年生存率91%(74/81),5年生存率84%(68/81)。结论全喉切除颈廓清术是治疗喉癌颈部淋巴结转移的有效方法,正确的护理对疾病的预后、并发症的预防及提高生存率极其重要。Objective Summary of the Eradicate treatment clearance technology for the treatment of cervical cancer in throat in the clinical effect and after the main complication of osteoporosis nursing. Methods Retrospective analysis division on January 2000 and on January 2006.81 cases of fire victims,laryngeal cancer according to UICC TNM partition,T1N0M0 26 cases,T2N0M025 cases,T2N1M0 17 cases,T3N2M013 cases,and in accordance with throat cut wide radical neck dissection with bilateral surgical indications. Results The 81 sufferers after none instance death,all clinical cure of hospital discharge with visiting for 5 years,died 13,4 among those instances because of neck transfer refuse again surgical operation death,2 because of lung transfer death,2 because of lung unit infection death,3 died in apoplexy,2 cause of death non-avaible,3 years,the existence led 91%（74/81）,5 years,the existence led 84%（68/81）. Conclusions The whole cut-off throat is effective for throat cancer,the accurate nursing is very important fo paroxysm,prevention and exaltation existence rate of the complications.
