B超引导下经皮肝穿刺活检术的围手术期护理

摘　　要：目的探讨B超引导下采用Bard肝穿枪经皮肝穿刺活检术的围手术期护理经验。方法回顾性分析2010年8月-2011年4月53例慢性乙型病毒性肝炎患者,B超引导下经皮肝穿刺活检术的围手术期临床观察和护理,包括做好术前准备、心理护理,指导术中密切配合、术后严密观察病情变化,及时发现并发症并采取措施。结果 53例患者B超引导下经皮肝穿刺活检术成功率100%,出现局部切口疼痛28例,胸闷、心悸2例。经采取适当的护理措施后缓解,无一例出现严重并发症。结论围手术期护理在肝穿刺术中具有重要作用,完善术前准备,做好心理护理,指导术中配合,采取舒适体位,术后密切观察病情变化是安全实施肝穿刺活检术的关键。Objective To explore the perioperative nursing experience in liver biopsy with Bard biopsy instrument under the guidance of B mode ultrasound.Methods We analyze retrospectively the perioperative clinical observation and nursing care in liver biopsy under the guidance of B mode ultrasound for 53 chronic hepatitis B patients from August 2010 to April 2011.The preoperative preparation and mental intervention was given to all patients undergoing the percutaneous liver biopsy.The closely cooperation in operation and the strictly postoperative monitoring of vital signs was conducted.The complications were discovered and treated early.Results The liver punctures were successfully completed in 53 patients under the guidance of type B ultrasound;No serious postoperative complications occurred.There were 28 patients with local pain around puncture points and 3 patients with chest distress,palpitations,all of them were relieved by the appropriate nursing.Conclusion The perioperative nursing care played an important role in liver biopsy under the guidance of B mode ultrasound,including mental care,preoperative preparation,intraoperative guidance,comfortable body position.The carefully observation on symptoms and signs can ensure the postoperative safe.
