参麦注射液联合无创正压通气治疗COPD呼吸衰竭的临床观察

摘　　要：目的:观察参麦注射液联合无创正压通气治疗慢性阻塞性肺疾病(COPD)合并严重呼吸衰竭的临床疗效。方法:将我院61例COPD合并呼吸衰竭患者随机分为对照组(31例)和治疗组(30例)。对照组在常规治疗的同时给予无创正压通气,治疗组在对照组基础上加用参麦注射液40mL加入5%葡萄糖注射液250mL中,静脉滴注,qd,共10d。观察2组治疗前、治疗后72h的动脉血气分析、C反应蛋白、急性生理和慢性健康状态评价系统Ⅱ评分;比较2组治疗有效率、住院时间和正压通气时间。结果:治疗组72h动脉血气分析结果、C反应蛋白优于对照组;治疗组治疗有效率高于对照组;治疗组无创正压通气时间(5.9±3.0)d少于对照组(7.5±3.6)d;治疗组住院时间(11.0±1.8)d少于对照组(13.6±2.5)d。2组比较差异均有统计学意义(P<0.05)。2组均未见明显不良反应。结论:参麦注射液联合无创正压通气治疗COPD呼吸衰竭能有效缓解病情,缩短住院时间和机械通气时间。To investigate the therapeutic efficacy of non-invasive mechanical ventilation （NPPV） combined with Shenmai injection in the treatment of COPD complicating with respiratory failure. METHODS： 61 COPD patients with respiratory failure were randomly divided into 2 groups： control group （n=31） and treatment group （n=30）. Both group were treated with conventional therapy plus NPPV, and treatment group additionally received Shenmai injection 40 mL added into 5% Glucose injec- tion 250 mL via i.v. once a day for 10 days. The arterial blood gas values, C-reactive protein and APACHE II score were com- pared between 2 groups before and after treatment. The effective rate, the duration of total mechanical ventilation and the duration of hospital stay were compared between 2 groups. RESULTS： The arterial blood gas values and C-reactive protein in the treatment group were better than control group. The effective rate of the treatment group were higher than the control group. The duration of hospitalization and mechanical ventilation were （11.0 ＋ 1.8） d and （5.9 ＋ 3.0） d in the treatment group, which were lower than （13.6 ＋ 2.5） d and （7.5 ＋ 3.6） d in the control group, there were significant differences between 2 groups （P〈0.05）.There were no serious ADRs found. CONCLUSION： Shenmai injection combined with NPPV could relieve disease in the treatment of COPD com- olicatin~ with resoiratorv failure and shorten the duration of hosaitalization and mechanical ventilation.
