摘  要:目的:验证膨体聚四氟乙烯缝线在心脏瓣膜成形术中行腱索替换的效果。方法:1991 年6 月至1998 年3 月在心瓣膜成形术中采用膨体聚四氟乙烯缝线进行人工腱索替换23 例:替换二尖瓣腱索28 支,三尖瓣腱索1 支。结果:病人全部康复出院。其中17 例随访2 ~78 个月,平均18 .5 个月。彩色B 超复查显示瓣膜关闭不全完全矫正者6 例,仍残存极轻度关闭不全者3 例,轻度关闭不全7 例,术后仍有中度反流1 例,系人工腱索过短所致。结论:膨体聚四氟乙烯缝线替换腱索方法简便、可靠,但术中须准确选定人工腱索长度及矫治好瓣膜其它病变。Aim:To evalute the method of expanded polytetrafluoroethylene(ePTFE) suture as artificial chordae in valvuloplasty.Clinical material and method:From June 1991 to March 1998,23 patients,12 males and 11 females,with a mean age of 21.8 years(range 1 to 57 years) underwent chordal replacement.22 cases had moderate to severe mitral valve insufficiency,1 had tricuspid chordal lesion caused by right atrial myxoma.Total 29 ePTFE sutures were implanted(28 for mitral valves,1 for tricuspid valve),and other traditional procedures for valve repair were performed.Result:There were no operative deaths or serious complications.Only one case was switched to MVR intraoperatively.17 cases were followed up from 2-78 months(mean 18.5 months).Color echocardiogram showed the valve was competent in 6,minimal regurgitation in 3,mild in 7 and moderate in 1.Conclusion:The ePTFE suture as an artificial chordae is a safe,practical and reliable technique in valvuloplasty.
