年龄对全盆底重建联合抗压力性尿失禁手术后患者主观感觉的影响

摘　　要：目的:探讨盆底功能障碍患者行全盆底重建联合抗压力性尿失禁手术后年龄对患者主观感觉及生活质量的影响。方法:回顾性分析61例年龄<65岁和35例年龄≥65岁同时进行了TVT-O和PROLIFT手术患者的临床资料,对术前及术后生活质量评分对比研究,分析年龄对术后效果的影响。结果:平均随访时间24.2个月,两组患者术后症状和生活质量均明显提高,术后两组均有PFDI-20和PFIQ-7直肠及排尿功能评分整体及独自的提高;≥65岁组与<65岁组相比有略低的PFDI-20评分提高,平均提高63±31;两组PFIQ-7评分整体提高无显著差异,≥65岁组30.8±42,<65岁组49.8±60。结论:65岁或更年长妇女经历了SUI联合POP的手术,有更高的再发SUI风险,但在症状与生活质量方面仍有很大提高。Objective：To explore the effect of age on the subjective feelings and quality of life of patients with pelvic floor dysfunction after total pelvic floor reconstruction combined with stress-urinary-incontinence surgery.Methods：The clinical data of 61 cases（65 years） and 35 cases（≥65 years） who received TVT-O and PROLIFT at the same time were analyzed retrospectively,the scores of quality of life before and after surgery were compared and researched,the effect of age on postoperative efficacy was analyzed.Results：The average follow-up time was 24.2 months.In the two groups,the postoperative symptoms and qualities of life were improved significantly;after surgery,the total score and single score of rectum and urinary function displayed by PFDI-20 and PFIQ-7 increased in the two groups;compared with 65-year group,the score of PFDI-20 in ≥65-year group increased by（63±31）;there was no significant difference in the increasing extent of total score of PFIQ-7 between ≥65-year group（30.8±42） and 65-year group（49.8±60）.Conclusion：The ≥65-year-old women who received total pelvic floor reconstruction combined with stress-urinary-incontinence surgery have higher recurrence risk of stress urinary incontinence,but the symptoms and quality of life are improved significantly.
