乙型肝炎血清性激素水平变化的临床研究

摘　　要：目的　探讨病毒性乙型肝炎患者血清中性激素水平的变化的临床意义。方法　选择７７ 例病毒性乙型肝炎（ 包括急性、慢性、重型和肝硬化） 患者以及２０ 例正常人，采用放免法检测其血清中的睾酮（Ｔ） ，雌二醇（Ｅ２） 、卵泡刺激素（ＦＳＨ） 和黄体生成素（ＬＨ） 的水平，并对病人临床资料加以收集分析。结果　临床症状上性欲减退以急性、重型肝炎和肝硬化病人多见，而性功能下降则以重型肝炎为多见，性激素检测显示：急性肝炎主要为Ｔ 水平下降，重型肝炎则表现为：Ｔ、ＦＳＨ、ＬＨ 均明显下降，而慢性乙肝和肝硬化则为Ｅ２ 水平升高，上述指标与正常对照组相比较，均有显著性或极其显著性差异（ Ｐ＜ ０ ．０５ 或Ｐ＜ ０ ．０１） ，同时急性乙肝的急性期Ｔ 水平下降，而在恢复期Ｔ水平也可恢复至正常，两者比较差异显著（ Ｐ＜ ０ ．０５） 。结论　病毒性乙型肝炎出现的性功能下降等症状与肝炎所致血清中性激素紊乱密切相关。Objective\ To explore the clinical significance of sex hormone in the sera of hepatitis B patients.Methods\ 77 cases of hepatitis B,including acute hepatitis B,fulminant hepatitis B,chronic hepatitis B and cirrhosis,and 20 cases of normal person were grouped as experimental group and control one.Radioimmunassay were usesd to detect the level of testosterone(T),estradiol(E2),follicle stimulating hormone (FSH) and luteinizing hormone(LH)in sera of hepatitis B.Results\ The level of T was significantly lower in acute and fulminant hepatitis B.But in chronic hepatitis B and cirrhosis,only the level of E2 was abnormally elevated.Comparing with that in control group,all of these had significantly difference(P<0.05 or P<0.01).Conclusion\ Hepatitis B can result in the disturbance of sex hormone,which is one of the important factors for the appearance of sexual dysfunction.
