摘  要:目的 探讨人工肝支持系统对重症肝炎的疗效。方法 应用日产KM —8800 血液净化仪对43 例重症病毒性肝炎选用不同的方法( 血浆交换、血液灌流、血浆吸附等) 进行102 次治疗,与对照组44 例比较。结果 明显急性、亚急性重症肝炎治愈好转率(65 .22 % ) 明显高于对照组(20 .83 % ) ;慢性重症肝炎治疗组治愈好转率(55 .00 % ) ;治疗组高于对照组(15 .00 % ) ;两组有显著性差异( P< 0 .01) ;治疗组每次治疗后患者乏力、纳差、腹胀等症状明显改善,精神好转,肝功能血胆红素、AST 等明显降低( P< 0 .001) ;血白蛋白升高( P< 0 .01) ,球蛋白明显降低( P<0 .001) ,白球比例由倒置转为正常或接近正常范围。血BUN、Cr、NH3 等治疗后均有明显降低( 分别为( P< 0 .001 ,P< 0 .05and P< 0 .001) ,电解质治疗前后无显著性差异,均为正常范围。结论 ALSS 不失为目前治疗重症肝炎有效、安全的治疗手段,能显著地降低病死率,提高成活率。Objective\ Tassess the curative effects of ALSS treatment of fatal hepatitis.Methods\ Use KM-8800 plaxma exchanger made in Japan to conduct 102 time of treatment on 43 fatal viral hepatitis cases through different ways(PE,PP and DHP),and then compared with 44 cases in control group.Results\ The curative rate of acute and sub-acute fatal hepatitis in treatment group (65.22%)is obviously higher than the control group(20.83%),and that of chronic fatal hepatitis in treatment group(55.00%) is obriously higher than the control group (15.00%),showing a significant difference (P<0.01)between the two groups.In the treatment group,the symtoms of hypodynamia,inappetence and abdominal distending take were effectively improved.The blood level of bilirubin,AST and globulin show significant decrease (P<0.001)inerease of blood albumin (P<0.01).The BUN,Cr and NH\-3 were also obviously reduced (P<0.001,P<0.005 and P<0.0001 respectively) after the treatment.Conclusion\ ALSS is indeed an effective and safe procedure to the treatment of fatal hepatitis,it can reduce the mortality rate and increase the survival rate.
