摘  要:目的:探讨抗细胞间黏附分子-1(ICAM-1)单克隆抗体对血管球囊拉伤术后血管重塑的影响作用。方法:制备抗ICAM-1单克隆抗体,将30只SD大鼠随机分为抗体组、手术对照组及正常对照组(各10只),每组又分为24h观察组(5只)和14d观察组(5只),抗体组通过尾静脉输入抗ICAM-1单克隆抗体观察球囊拉伤后的大鼠股动脉内壁的变化,应用免疫组织化学、光镜等方法检测拉伤局部内膜变化情况。结果:抗体组球囊拉伤血管24h后内膜处有极少量中性粒细胞浸润,而手术对照组损伤内膜有大量的中性粒细胞浸润,14d后抗体组球囊拉伤血管内膜厚度小于手术对照组,管腔内径大于手术对照组,且内膜有较多VEGF蛋白颗粒表达。结论:病理性重塑是血管损伤后再狭窄的主要原因,由于损伤致周围组织炎性改变,致使大量白细胞向损伤局部内膜浸润,加重内膜损伤,而抗ICAM-1单克隆抗体可对抗白细胞与受损内膜的内皮细胞结合,减轻损伤血管内膜的炎性反应,对防治再狭窄有一定作用。Objective: To investigate the mechanism of vascular reconstruction and monoclonal antibody to interceller adhesion molecule-1 to prevent the restenosis caused by balloon angioplasty.Method: 30 SD rats were divided into 3 groups.That monoclonal antibody to interceller adhesion molecule-1 were infused into the tail vein of rats was one group.The other group was operation control group,The third group used as normal group.Each group was divided into two groups again.One group(24h group) were killed after 24 hours and another group(14 d group) were killed after 14 days and examined for VEGF protein expression by immunohistochemical techniqe.Polymorphonuclears(PMN) were counted in the injury-artery intima.The thickness of artery intima was measured and we examined intima with scanning eletromicroscope and light microscope.Result: In 24h group the infiltration of PMN was low in the first group.In 14d group VEGF protein expression was higher in group which were received monoclonal antibody to interceller adhesion molecule-1 than operation control group and normal group.Conclusion: Monoclonal antibody to interceller adhesion molecule-1 can decrease the infiltration of PMN,stimulate the proliferation and accelerate the endotheliazation of injury-artery.
