经颅多普勒评价交感型颈椎病疗效的意义

摘　　要：目的:分析交感型颈椎病治疗前后的经颅多普勒(TCD)变化,探讨TCD评价交感型颈椎病疗效的临床意义。方法:在2008年7月—2010年3月期间,收集住院后TCD检查异常的交感型颈椎病患者29例,给予推拿、中频、中药热敷、牵引、骶管冲击、小针刀等综合保守治疗,治疗后评价症状恢复情况;并且复查TCD,分析TCD变化与疗效变化是否具有相关性。结果:复查TCD恢复正常26例(优17例,良7例,有效2例);改善3例(优0例,良1例,有效2例);TCD改变与疗效相关性分析:r=0.524,t=3.194,P<0.01。结论:TCD改变与疗效呈正相关,可作为交感型颈椎病疗效评价的参考指标,且有经济实惠、无创伤、可重复等优点。Objective：To analyse the changes on transcranial doppler（TCD） with the patients who suffer from sympathetic cervical spondylosis before and after treatment,and to discuss the clinical significance of TCD in the evaluation of the curative effect of sympathetic cervical spondylosis.Methods：From 2008-07 to 2010-03,29 sympathetic cervical spondylosis inpatients with abnormal TCD results were collected and treated by comprehensively conservative treatments,including massage,computer IF,Chinese medicine fomentation,traction,sacral injection,small needle scalpel and so on.After treatment,to evaluate the recovery of the symptoms;then the patients were checked by TCD again,all the results were analysed if there was the relationship between the changes of TCD results and the changes of curative effects.Results：26 cases＇ TCD results returned to normal（17 patients healed excellent,7 patients improved partially,2 patients turned resultful）;and 3 cases＇ TCD results improved（0 patients healed excellent,1 patients improved partially,2 patients turned resultful）;the analyses of correlation between the changes of TCD results and the curative effects：r=0.524,t=3.194,P0.01.Conclusion：The changes of TCD results and the curative effects have positive correlation,and TCD can be used as the reference index in the evaluation of the curative effect of sympathetic cervical spondylosis and it takes the advantage of affordable,non-invasive,repeatable and so on.
