基于文献研究的肝郁脾虚证临床诊治规律探析

摘　　要：目的:探析肝郁脾虚证临床诊治规律。方法:通过对1999年-2009年期间有关肝郁脾虚证在临床报道方面文献整理、归纳,统计并分析肝郁脾虚证的相关疾病病种、证型诊断标准、基本方及药物配伍、临床疗效等内容,以期探析肝郁脾虚证临床诊治规律。结果:666篇文献中肝郁脾虚证涉及疾病73种,对其中83篇文献临床治疗效果进行统计,证型的诊断多采用国家标准、行业标准或自拟。治疗以疏肝健脾为主的基本方加减化裁,或辅以西药联合用药,或配合穴位敷贴、推拿加温灸等方法。总样本量达7 481例,治疗组4 455例总有效率92.1%。结论:在相关疾病病种中,肝郁脾虚证出现在肝病、胃部疾病和肠道疾病的频次最高,逍遥散仍是治疗肝郁脾虚证的代表方,而白术、柴胡、茯苓、白芍、甘草使用频率高于其他药物且差异有统计学意义。Objective： To analyze the regularity of clinical diagnosis and treatment about stagnation of Liver Qi and spleen deficiency pattern.Methods： According to sorting,summarizing,analyzing literature of diseases types,pattern diagnosis standard,basic prescription,drugs compatibility and clinical curative effects which related to stagnation of Liver Qi and spleen deficiency pattern,to investigate rules of clinical diagnosis and treatment on this pattern of syndrome.Results： There were 73 diseases refering to this type of syndrome in 666 documents.we selected 83 articles and carried out statistic of clinical curative effects of those.Among these documents,variousdiagnosis criteria were used which contained national criteria,professional standard and draft criteria by oneself.The treatment method is modification of the basic formula dominating on dispersing the stagnation of Liver Qi and invigorating spleen,sometimes assisting traditional medicine with western medicine,or combining acupoint application and massage adding warming-needle moxibustion.Total sample size are 7 481 cases,and total effective rate of treatment group is 92.1%.Conclusion： Among those correlating diseases,frequency of stagnation of Liver Qi and spleen deficiency pattern is highest in Liver,stomach and intestinal tract diseases.Xiaoyao San remains representative formula of stagnation of Liver Qi and spleen deficiency pattern.The frequencies of Rhizoma Atractylodis Macrocephalae,Radix Bupleuri,Poria,Radix Paeoniae Alba,Radix et Rhizoma Glycyrrhizae are higher than other herbs and differences are significant.
