摘 要:目的:探讨加味小青龙汤对脂多糖(LPS)诱导的急性肺损伤(ALI)大鼠肺组织Toll样受体4(TLR4)mRNA表达的影响。方法:将72只雄性Wistar大鼠随机分为:对照组、模型组、地塞米松组和加味小青龙汤(小、中、大剂量)组,每组再分为4h和8h两个亚组,每个亚组6只。尾静脉注射LPS 10mg/kg制备大鼠ALI模型。地塞米松组和加味小青龙汤(小、中、大剂量)组分别于造模前灌胃地塞米松和加味小青龙汤,2次/d,3d。采用实时荧光定量聚合酶链反应(PCR)检测TLR4 mRNA的表达,并于光镜下观察大鼠肺组织病理变化。结果:与模型组相比,在8h组中,加味小青龙汤(小、大剂量)组的TLR4 mRNA表达均明显降低(P<0.01)。病理学观察,模型组大鼠肺组织出现大片出血及坏死,各治疗组大鼠肺组织病理改变明显轻于模型组,肺细支气管偶见炎症改变,水肿较轻,中性粒细胞(PMN)和红细胞渗出较少。结论:加味小青龙汤能减轻内毒素致ALI大鼠肺组织损伤,对肺损伤有保护作用,其机制可能与其能降低内毒素致ALI大鼠肺组织TLR4 mRNA的表达有关。Objective: To investigate the effects of Jiawei Xiaoqinglong Decoction on lung of expressions of TLR4mRNA in the rats with acute lung injury caused by lipopolysaccharide(LPS).Methods: Seventy-two male Wistar rats were randomly divided into six groups including normal control,LPS model,dexamethasone and Jiawei Xiaoqinglong Decoction(Small,medium and large doses) groups.Each group had two subgroups,4h and 8h after LPS were injected.Each subgroup had six rats.The ALI rat model was established by LPS through intravenous tail injection(10mg/kg).The rats in dexamethasone group or Jiawei Xiaoqinglong Decoction group were pretreated by dexamethasone or Jiawei Xiaoqinglong Decoction(Small,medium and large doses) respectively for 3 days before LPS-induced ALI.The TLR4 mRNA was measured by real-time fluorescent Polymerase chain reaction(PCR) while the histopathology of the lung injury was observed by light microscope.Results: Compared with model group,the expression of TLR4 mRNA were obviously decreased in Jiawei Xiaoqinglong Decoction(small and large doses) groups at 8 hour(P0.01).Light microscope observation indicates that there were large areas of pulmonary hemorrhage and necrosis in model group,while in the Jiawei Xiaoqinglong Decoction group,the pathological manifestations were much more ameliorated than those of the model group.The lung bronchiole may inflammte sometimes,and the edema is lightly.PMN and RBC leak little.Conclusion: Jiawei Xiaoqinglong Decoction lessens the injury of lung tissue and has protective effects on rats with ALI induced by LPS,the mechanism is possibly related to the inhibition of the expressions of TLR4 mRNA in injured lung tissues.
