摘  要:目的观察脑出血及颅脑外伤后昏迷患者呼吸紊乱的类型、程度及与格拉斯哥昏迷评分(GCS)的关系。方法对2例脑外伤、2例脑出血昏迷患者在入院当天、1周后及2周后进行睡眠呼吸监测,然后将睡眠呼吸监测相关指标与患者的意识状态和GCS评分进行对照。结果 4例患者均存在严重的呼吸暂停,包括中枢性呼吸暂停和阻塞性呼吸暂停。4例昏迷患者的呼吸暂停程度与其GCS评分均呈正相关。结论脑外伤和脑出血昏迷患者并存着中枢性呼吸暂停和阻塞性呼吸暂停,但在不同的患者,中枢性呼吸暂停和阻塞性呼吸暂停有不同程度的差异。脑外伤和脑出血昏迷患者呼吸暂停的严重程度与患者GCS评分呈正相关。Objective To observe of apnea and hypopnea in coma patients due to intracranial lesions and the relationship between apnea and glasgow coma scale (GCS). Methods Polysomnography was applied and recorded in coma patients in neurosurgical wards on admission, 1 week after admission, 2 weeks after admission. Patients were also submitted to clinical examination with determination of the GCS score and consciousness. Results Serious apnea and hypopnea were found in all of the 4 patients. All of the 4 patients who had better improvement of GCS scores had a better improvement of respiratory situation. Conclusions Comatose patients due to intracranial lesions have central apnea and obstructive apnea. Some of the patients have serious central apnea and the others may have more serious obstructive apnea. The severity of apnea and GCS scores of coma patients are positively correlated.
