摘  要:目的了解偏瘫恢复期患者的平衡能力和姿势对称性。方法偏瘫患者(均具备步行能力)和健康人各24例,采用意大利tecnobody公司生产的PK254P平衡反馈训练仪测定患者的平衡能力[压力中心(COP)前后标准差、左右标准差、前后平均运动速度、左右平均运动速度、运动长度、运动椭圆面积]和对称性(X轴平均COP),分别在标准静态坐位及站立位下测试30S。结果偏瘫患者在静态坐位和站立位下的COP前后标准差、左右标准差、前后平均运动速度、左右平均运动速度、运动长度、运动椭圆面积均高于正常对照组(P〈0.05),偏瘫患者的X轴平均COP大于正常对照组(P〈0.05)。X轴平均COP与反应平衡能力的6个参数之间具有一定的相关性(r=0.44~0.68)。结论PK254P平衡反馈训练仪对偏瘫患者坐位和站立位下的平衡能力的检测具有很好的敏感性。偏瘫患者即使具备步行能力,但其躯干控制能力与站立位下的平衡能力较差,所以对具备步行能力的偏瘫患者仍需进行平衡训练以进一步提高其生活质量和行走能力。静态站立位下偏瘫患者体质量分布不对称,其重心仍偏向健侧;偏瘫患者的体重分布不对称性越严重,平衡能力越差。Objective To study the balance capability and postural symmetry in chronic hemiparetic patients with brain injury. Methods Balance and postural symmetry of 24 hemiparetic patients and 24 healthy control subjects were assessed by PK 254 P rehabilitation system of standard sitting and standing for 30 s. The variables of anterior-posterior(AP) standard deviation of center of pressure(COP) , medial-lateral (ML) standard deviation of COP, average AP speed of COP, average ML speed of COP, perimeter of COP, and ellipse area of COP were used in determining the balance. Also the average position of COP in X axis represented the symmetry of weight distribution in standing. Results In hemiparetic both sitting and standing (AP standard deviation of COP, ML standard deviation of COP, average AP speed of COP, average ML speed of COP, perimeter of COP, and ehpse area of COP) were lager than health subjects( P 〈 0.05 ), and the average position of COP in hemiparetic was bigger than control subjects( P 〈 0.05 ). There was moderate correlations ( r = 0.44-0.68 ) between measures of average position of COP and other six postural stability variables. Conclusions PK 254P balancing feedback apparatus is sensitive to test hemiparetic patients balance in sitting and standing positions. For hemiparetic patients with ambulation capability, there were still trunk control and balance disturbances. These patients with the capability of walking still need balance or postural control training to further improve their daily livings and walking performances. The hemiparetic patients presents asymmetrial weight distribution, and center of mass moved toward the nonparetic side. The more asymmetrical the weight bearing is, the worse the balance.
