吞咽障碍患者特征性表现与吞咽X线荧光透视检查的对应关系

摘　　要：目的探讨吞咽障碍患者特征性临床表现与吞咽X线荧光透视检查（VFSS）观察所见的对应关系。方法收集2008年6月至2011年3月郑州大学第五附属医院康复中心收治的120例吞咽障碍患者，分别对其进行1ml液态，10ml液态，10m1糊状，1／4块固体食物（包钡蛋糕食10cm^3）4种食物质地的测试，完成临床评定和VFSS，总结吞咽障碍患者特征性表现与VFSS观察所见障碍点对应关系。结果评定吞咽功能所选择的食物应为10ml液体或10cm^3固体，120例患者共进行460次吞咽，在口腔期和咽期特征性的临床表现与吞咽X线荧光透视检查所见r值在0．645—0．804，P〈0．01，呈显著相关性。结论吞咽障碍患者特征性表现与VFSS有着一定的对应规律，此规律能更简捷、准确、安全地判断患者吞咽障碍的产生原因。Objective To explore the corresponding relations between the clinic marks of deglutition disorders and that of videofluoroscopic swallowing study （ VFSS ）. Thus we may create a more convenient and volant training ways based on the corrrespongding relations. Methods One hundred and twenty patients with deglutition disorders were selected accepting swallowing function observation and VFSS having 1 ml liquid diet, 10 ml liquid diet, 10 ml pasty diet, 10 cm^3 solid diet respectively. Meanwhile, the corresponding relations between the clinic marks of deglutition disorders and that of VFSS were summarized. Results 10 ml liquid diet or 10 cm^3 solid diet should be selected to evaluate swallowing functions, and 460 times swallowing tests were carried out for 120 pa- tients. The clinic marks of deglutition disorders had good consistency with that of VFSS in oral and pharynx stages, with r value being 0. 645 to 0. 804, P value being not so many as 0.01. Conclusions The corresponding relations exists between clinic marks and that of VFSS, which contribute to find the internal reasons of deglutition disorders more briefly and exactly.
