成人不典型麻诊96例临床分析

摘　　要：目的通过探讨成人不典型麻疹的临床特点，进而早期积极预防避免其流行。方法回顾性分析96例成人不典型麻疹的临床资料，总结临床表现、发疹时间、并发症、实验室血常规、肝功及麻疹IgM检查，并进行统计学分析。结果成人不典型麻疹临床表现以发热、不典型皮疹、腹泻、流感样症状、结膜充血、麻疹粘膜斑等较为常见，症状以发热最多见，体征以结膜充血最明显；出疹时间以第5天所占比例最大，同其他时间相比差异有显著性（P〈0．05）并发症以支气管肺炎最多见，同其他并发症相比差异有显著性（P〈0．05）WBC正常者17．71％、升高者58．33％、降低者23．96％；ALT升高者41．67％；AST升高者45．83％；麻疹IgM阳性病例92．71％，阴性病例7．29％。结论对于成人麻疹尤其不典型者，临床医师应通过常见不典型症状并结合实验室检查早期诊断、积极预防以避免其流行传播。Objective To explore the clinical features of adult non-typical measles to avoid epidemic by early effort. Methods Clinical data of 96 adult patients with atypical measles were retrospec- tively analyzed, summing up the clinical manifestations, rash time, complications, laboratory blood, liver and measles IgM inspection statistically. Results Fever, atypical rash, diarrhea, flu-like symptoms, conjunctival hyperemia, measles and common mucosal spots were common clinical manifestations of adult atypical measles, and the most common symptom was fever, with conjunctival hyperemia as the most obvious sign the most frequently rash time was the fifth day compared with the other time （P〈 0.05） the most common complication was bronchial pneumonia compared with the other complications （P〈 0.05）; the cases with normal WBC were 17.71%, 58.33% increased; 23.96% lowered. ALT increased was 41.67%; AST 45.83% increased; measles IgM positive cases were 92.71%, 7.29% negative. Conclusion The clinieists should study the clinical features of adult non-typical measles and combine the common normal to avoid epidemic laboratory tests, providing early diagnosis and avoiding its epidemic spreading.
