难治性癫痫中医证候聚类分析

摘　　要：目的应用变量聚类分析的数理统计方法,对难治性癫痫进行中医分型。方法收集难治性癫痫患者76例,进行中医四诊检查,将症状分为轻度、中度、重度和严重4级,按照中医辨证记分标准逐一记分。对采集的证候要素进行频数统计,对变量进行变量聚类分析。结果难治性癫痫在调查群体中分为4型。肾虚精亏(37.6%):智力减退、健忘、肢冷、动作迟钝、腰酸、形体痿软、头脑空痛、尿频、虚脉;肝郁痰凝(29.8%):心烦、抑郁、口干、恶心、口臭、口黏、身体困重、舌红;心脾两虚(21.1%):失眠、神疲、乏力、易恐、易思、畏寒、胆怯、细脉;气滞血瘀(11.5%):头痛、急躁、易怒、弦脉、舌黯。变量解释比例为68.93%。结论难治性癫痫的中医证候比较复杂,而变量聚类分析能够帮助难治性癫痫中医证候的合理分型。Objective To analyse the TCM syndrome of intractable epilepsy, by the way of variable cluster analysis. Methods We researched 76 intractable epilepsy patients, and recorded their symptoms, tongues and pulses. The patients＇ symptoms were evaluated as mild, moderate, severe and extreme severe, and recorded according to standard scale of TCM syndrome. The patients＇ clinical data were analyzed by cluster analysis. Results These patients could be divided into 4 types. Marrow and kidney deficiency syndrome （37.6%）： hypophrenia, forgettery, cold limbs, lumbering, mild lumbago, atrophy body, empty headache, frequent micturition, feeble pluse. Liver qi and phlegm stasis syndrome （29.8%）： vexation, depression, dry mouth, surfeit, halitosis, sticky mouth, stranded body, red tongue. Heart and spleen deficiency syndrome （21.1%）： insomnia, fatigue, weakness, fear, yearn, chilly, timidity, thready pulse. Qi and blood stasis syndrome （11.5%）： headache, impetuosity, irascibility, dark tongue, string pulse. The variable proportion is 68.93%. Conclusion The symptoms of intractable epilepsy were very complex, and variable cluster analysis can help TCM syndrome differentiation of intractable epilepsy.
