摘  要:目的应用世界卫生组织骨折风险因子评估工具(FRAX)识别绝经后女性骨质疏松症。方法在秦皇岛市第一医院进行骨密度测量的绝经后女性103名为研究对象,平均年龄(64.1±9.8)岁。采用问卷调查的方法,计算骨折风险(不应用骨密度)和亚洲人骨质疏松自我筛查工具(OSTA)指数。结果共检出骨质疏松症患者43例,骨质疏松组年龄明显高于非骨质疏松组(P<0.01)。骨质疏松组OSTA指数明显低于非骨质疏松组,10年髋部骨折风险和主要骨质疏松性骨折风险明显高于非骨质疏松组(P<0.01)。OSTA指数识别骨质疏松症的ROC曲线下面积为0.734(P=0.000),准确性中等。10年髋部骨折风险识别骨质疏松症的ROC曲线下面积为0.766(P=0.000),准确性中等。结论 FRAX工具同样可用于识别绝经后女性骨质疏松症,但并不优于OSTA指数。Objective The aim of this study was to analyze the discrimination of fracture risk assessment tool in the assessment of osteoporosis in postmenopausal women. Methods 103 postmenopausal women (64. 1 ± 9. 8 years), were recruited from the First Hospital of Qinhuangdao. Questionnaire survey was conducted. Bone mineral density was measured by dual energy x - ray absorptiometry. 10 - year probabilities of major fracture and hip fracture were calculated by the fracture risk assessment tool (without bone mineral density). The index of osteoporosis self - assessment tool for Asians was also calculated. Results The age was old in osteoporotic group (P 〈 0. 01 ) . The level of OSTA was low in fracture group (P 〈 0.01 ). 10 - year probabilities of major fracture and hip fracture were higher in osteoporutic group (P 〈 0. 01 ). Receiver operating characteristic curve analysis also showed that OSTA ( Area under the curve : 0. 734, P = 0. 000) and 10 -year probabilities of hip fracture (Area under the curve: 0. 766, P =0. 000) could identify osteoporosis. Conclusion Fracture risk evaluated by FRAX may be helpful to identify osteoporosis in postmenopausal women, but was not superior to OSTA.
