慢性鼻及鼻窦炎患者主客观嗅功能检查的相关性分析

摘　　要：目的探讨慢性鼻及鼻窦炎(CRS)致嗅觉障碍患者主观嗅功能(T&T嗅觉检查法)和嗅觉诱发电位(olfactory event related potential,OERP)的相关性。方法 89例CRS患者按有无鼻息肉分成两组,记录年龄、性别、症状、鼻窦CT、鼻内镜检查、主观嗅觉功能和OERP。结果非鼻息肉组T&T得分与OERP的N1、P2潜伏期及幅值相关(P<0.05),鼻息肉组主客观检查结果不相关(P>0.05)。对鼻息肉组进一步分组发现,部分患者主观嗅觉功能完全丧失(T&T≥5.6),OERP存在;鼻息肉组主观嗅觉功能非完全丧失者(T&T<5.6)主客观检查显著相关(P<0.05)。结论 OERP可以应用于CRS致嗅觉障碍患者嗅觉功能的检查。嗅裂特殊部位的阻塞会影响主客观检查一致性。OBJECTIVE To evaluate the consistency between olfactory event-related potentials （OERP） and psychophysics olfactory test（TT smell test） in rhinology clinic.METHODS This study encompassed a cohort of 89 patients with a complaint of olfactory dysfunction for chronic rhinosinusitis.All the patients were divided into two groups according to whether polyps existed or not judged from endoscopic examination and CT scan.Information included age, gender,symptoms,sinus CT,endoscopic examination, olfactory event related potential,psychophysics olfactory test were recorded in detail.RESULTS TT results of patients without polyps correlated with latency and amplitude of N1 and P2（two components of OERP, P0.05）.No significant correlation existed in polyps patients between psychophysics olfactory function and four components of OERP（P0.05）.Patients with polyps were sub-divided into two groups,polyp patients with free olfactory cleft and polyp patients with polyps partially obstructed olfactory cleft.There was a significant correlation between TT scores and OERP of polyp patients with free olfactory cleft.For patients with polyps partially obstructed olfactory cleft with identified OERP, psychophysical olfactory was anosmia.CONCLUSION OERP can be used to investigate olfactory function in rhinosinusitis patients.Certain sites of olfactory cleft obstructed with polyps can influence the consistency between psychophysical olfactory test and OERP.
