全方向M型超声心动图评价2型糖尿病患者左室舒张功能40例临床观测

摘　　要：目的应用全方向M型超声心动图评价2型糖尿病患者的左室舒张功能。方法 2型糖尿病患者40例和健康志愿者30例,应用二维超声和全方向M型超声心动图检测左心房内径(LA)、左心室内径(LV)、二尖瓣口血流传播速度(Vp)、等容舒张时间(IVRT)、二尖瓣口血流E峰/二尖瓣口血流A峰(E/A)、左室射血分数(EF)和舒张期早期运动速度达峰时间(Td),并计算Td的标准差(Td-12-sd)及最大时间差(Td-12-dif)。结果糖尿病组的LA、IVRT大于对照组,Vp、A值、E/A比值小于对照组(P<0.05);左心室短轴二尖瓣水平糖尿病组前壁、下壁及后间隔的Td均大于对照组(P<0.01),左室短轴乳头肌水平糖尿病组各室壁的Td均大于对照组(P<0.05);糖尿病组Td-12-Sd大于正常组(P<0.05)。结论糖尿病患者在左室整体收缩功能尚未出现异常时,就已出现左室舒张功能的减退和舒张运动的不协调。全方向M型超声心动图可准确评价2型糖尿病患者的左室舒张功能。Objective To evaluate the changes of left ventricular diastolic function in patients with type 2 diabetes mellitus（DM） by omni-directional m-mode echocardiography（OME）.Methods 40 DM patients and 30 health volunteers were recruited in the present study,The left atrial diameter（LA）,left ventricular diameter（LV）,mitral flow propagation velocity（Vp）,isovolumic relaxation time（IVRT）,E/A,left ventricular ejection fraction（LVEF） and time to peak myocardial diastolic velocities（Td） were measured by two-dimensional echocardiography and OME.The standard deviation and maximum time difference of Td were calculated.Results Compared with the control group,LA and IVRT were increased,and Vp,A and E/A were decreased in DM group（P0.05）.Td of anterior wall,inferior wall and posterior septal of left ventricular short axis at the mitral valve level in DM group were longer than that of control group（P0.01）.Td of each segment of left ventricular short axis at the papillary muscle level in DM group were longer than that of control group（P0.05）.The standard deviation of Td in DM group was higher than that of control group（P0.05）.Conclusion Although the patients with type 2 DM have normal overall systolic function of left ventricle,they have left ventricular diastolic dysfunction already.OME can be used to evaluate accurately the left ventricular diastolic function in patients with type 2 DM.
