摘  要:[目的]通过对重症肺炎患儿血清心肌肌钙蛋白Ⅰ(cardiac troponinI,cTnI)和磷酸肌酸激酶同功酶(creatinephos phokinase-isoenzyme-MB,CK-MB)水平的检测,评价其在心肌损伤早期诊断中的价值。[方法]采用化学发光法和免疫抑制法检测61例合并心肌损伤的重症肺炎患儿及57例未合并心肌损伤的重症肺炎患儿血清cTnI和CK-MB水平。[结果]重症肺炎患儿心肌损伤组血清cTnI和CK-MB水平明显高于非心肌损伤组(P﹤0.05);cTnI在诊断重症肺炎合并心肌损伤的敏感度高于CK-MB;cTnI窗口时间明显宽于CK-MB。[结论]血清cTnI和CK-MB水平可作为早期诊断重症肺炎患儿心肌损伤的监测指标,且cTnI水平在诊断重症肺炎合并心肌损伤的临床价值更大。[Objective]To explore the value of serum cardiac troponin I(cTnI)and creatine phosphokinase-isoenzyme-MB(CK-MB)in early diagnosis of children’s severe pneumonia with myocardial injury by detecting the serum levels of cTnI and CK-MB.[Methods]The serum levels of cTnI and CK-MB were quantitatively detected by Chemiluminescence and immune suppression in 61 severe pneumonia children with myocardial injury and 57 severe pneumonia children without myocardial injury.[Results]Compared to patients without myocardial injury,the serum levels of cTnI and CK-MB of patients with myocardial injury of children’s severe pneumonia was significantly higher(P﹤0.05). The cTnI was more sensitive than CK-MB in the diagnosis of severe pneumonia with myocardial injury,and the window time of cTnI was obviously wider than CK-MB.[Conclusion]The serum levels of cTnI and CK-MB can be used as monitoring indicators in early diagnosis of children’s severe pneumonia with myocardial injury. The cTnI has higher clinical value in patients of children’s severe pneumonia with myocardial injury compared to CK-MB.
