子宫下段剖宫产术式的改良及其临床应用研究

摘　　要：目的　探讨剖宫产新术式,提高手术安全性。方法　选择有剖宫产指征、无下腹部手术史者1985例,随机分为两组,研究组725例,采用自行设计的改良经腹部子宫下段剖宫产术式;对照组1260例,603例采用传统的经腹腔子宫下段剖宫产术式,657例采用Stark’s剖宫产术式。结果　研究组手术时间、切皮至胎儿娩出时间、术后疼痛时间、肛门排气时间和卧床时间显著短于对照组的两种术式,出血量明显减少,新生儿Apgar评分高,刀口愈合好。结论　改良式剖宫产术能显著提高母婴安全性,术后病率低,操作简单快捷。Objective To investigate a new method of cesarean section in order to improve the safety of operation.Methods 1985 cases of pregnant women with cesarean indications and without any lower abdominal operation history were chosen and randomly divided into two groups.The study group(725 cases ) carried out a self-designed modified abdominal cervical cesarean section,while the control group(1260 cases) selected the traditional operation (603 cases)and Stark's cesarean section (657 cases).Results Not only the operative time,the time of postoperative pain,the time from incision to delivery,the time of anus exsufflation and the time of bed rest in the study group were significantly shorter than the time of both traditional operative methods in the control group,but also the bleeding was obviously decreased,the infant Apgar's score was higher and the incision was healed well.Conclusion Modified cesarean section can remarkably improve the safety of both mothers and babies and decrease the complications.And therefore,this is an easier and rapider technique to carry out.
