经阴道超声造影在宫颈癌诊断中的应用

摘　　要：目的探讨CEUS诊断宫颈癌及评估宫颈癌浸润范围的应用价值。方法选取宫颈浸润癌患者27例(Ⅰ、Ⅱ和Ⅲ期分别为12例、14例和1例),宫颈上皮内瘤变(CIN)Ⅲ患者13例,另选13例慢性宫颈炎患者,进行经阴道CEUS检查,观察并存储宫颈病变的增强过程,并对时间-强度曲线进行分析。结果注射造影剂后,慢性宫颈炎、CINⅢ及宫颈浸润癌的血流信号较造影前均明显增多。慢性宫颈炎与子宫体呈同步增强与消退。宫颈浸润癌病灶迅速增强,达峰时间明显缩短(P<0.05),可清晰显示病灶的范围、边界;之后缓慢消退。CINⅢ宫颈灌注早于子宫肌层,呈弥漫性高增强,后与宫体同步消退。结论 CEUS可直观显示宫颈病变的血流灌注。宫颈浸润癌的CEUS具有典型声像图表现,可为诊断及评估浸润范围提供更加丰富的信息。Objective To evaluate the value of transvaginal CEUS in diagnosis of cervical carcinoma and infiltrative range.Methods Twenty-seven patients with invasive cervical carcinoma（12 cases of stageⅠ,14 of stageⅡ and 1 case of stage Ⅲ）,13 patients with cervical intraepithelial neoplasia（CIN） Ⅲ and 13 patients with chronic cervicitis underwent transvaginal CEUS.The process of enhancement and washout were observed and stored,and the time-intensity curves were made.Results After the use of contrast agents,blood signal enhanced significantly around and inside the cervix.In patients with chronic cervicitis,the cervix enhanced simultaneously with the myometrium.In patients with invasive cervical carcinoma,the lesions enhanced homogeneously or heterogeneously earlier than the myometrium.Time to peak of invasive cervical carcinoma was shorter than the others（P0.05）.The extension and border of invasive cervical carcinoma could be determined clearly,and the process of washout was slow.In CIN Ⅲ,the cervix enhanced earlier than the myometrium,while washed out simultaneously with the myometrium.Conclusion Transvaginal CEUS is useful to investigate the vascular of the cervical lesions.It is valuable for the diagnosis of cervical carcinoma and the estimation of its scope and extension.
