血β2微球蛋白、血清胱抑素C及其他标志物的检测在高血压病肾损害诊断中的意义

摘　　要：【目的】探讨血β2微球蛋白（血β2—MG）、血清胱抑素C（Cys—C）及其他标志物在高血压病不同血压分级、年龄及病程中的临床诊断意义。【方法】随机选择原发性高血压患者170例，按血压程度、年龄、病程进行分组，随机选取同一时间的非高血压者为对照组，用全自动生化仪检测尿素氮（BUN）、肌酐（Cr）、血清Cys—C和血β2—MG浓度，比较各观察指标的阳性率和结果。【结果】各标志物均随血压分级升高呈不同升高趋势（P〈O．05），血β2—MG表现尤为显著（P〈0．05）。1级高血压中，血β2－MG阳性率高于其他标志物。血β2－MG、血Cys—C、Cr与血压分级有显著正相关关系（P〈0．01，P〈0．05），且其阳性率随病程延长而增加，各病程中，血pz—MG阳性率均最高（P〈0．01）。在50～60岁高血压患者中，血β2MG已反映出阳性，其余各标志物均在70～80岁开始呈现阳性率明显增长（P〈0．05）。【结论】血β2MG的检测是早期高血压肾损害诊断的重要指标，联合检测血Cys—C、β2－MG有助于发现早期高血压肾功能损害。[Objective] To investigate the clinical significance of serum132-microglobulin （β2-MG）, serum Cystatin C（Cys C）and other markers in Hypertension grading, age brackets and length of times. [Methods] Eighty-Tfive cases with Essential Hypertension were enrolled into the study and classified into three grades, four a; brackets and four length of times. Positive rates of four various markers measured by radioimmunoas-say were compared. [Results] With blood pressure going up, the tendency of all markers ascended, and positive rate of bloodβ2-MG became obvious（ P〈0.01）. In grade 1 hypertension, positive rates of bloodβ22-MG was higher than that of other markers. There was remarkable positive relationship among blood β2-MG, serum cystatin C, Cr and blood pressure levels. In hypertension cases, positive rate of blood β2-MG had been found when fifty to sixty years, other markers began to increase evidently when seventy to eighty years. In all length of times,blood β2-MG was the highest index. [Conclusion] Bloodβ2-MO is one of the useful markers to detect early renal dysfunction in hypertension. Combine detection of serum Cystatin C and blood β2-MG were helpful of finding early renal dysfunction in hypertension.
