胆囊床血管分布类型的彩色多普勒超声研究

摘　　要：目的:应用彩色多普勒超声全面评价胆囊床周围肝内血管的分布及走行情况。方法:应用彩色多普勒超声观察160例正常成人胆囊床周围2cm范围内的门静脉分支,肝动脉分支及肝静脉属支的内径、距胆囊床的距离及其分布类型。结果:受检者距胆囊床2cm范围的肝实质内均可观察到肝内血管的分布,肝静脉属支、门静脉分支及肝动脉分支的显示率分别为88.13%、90.00%及1.88%,其中紧贴于胆囊床的肝静脉属支与门静脉分支的显示率分别为18.13%、8.13%。胆囊床血管可分为肝静脉优势型、门静脉优势型及肝静脉-门静脉混合型三型,分别占58.75%、37.50%及3.75%。结论:彩色多普勒超声可无创性准确评价胆囊床血管的分布类型、内径及其与胆囊壁的关系。Objective：To evaluate the distribution types of the intrahepatic blood vessels around the gallbladder bed by color doppler ultrasound.Methods：The branches of hepatic veins,portal veins and hepatic arteries around the gallbladder bed were studied in 160 healthy volunteers,by using color Doppler ultrasound.The diameters,distances to the gallbladder wall,and distribution types of the vessels were mainly investigated.Results：Within the limit of 2 cm around the gallbladder bed,vessel branches from either hepatic veins（88.13%）,or portal veins（90.00%） and hepatic arteries（1.88%） could be demonstrated by color Doppler ultrasound.Among all,the rates of the hepatic vein branches and portal vein branches that were adjacent completely to gallbladder wall were respectively 18.13% and 8.13%.The distribution types of the blood vessels around the gallbladder bed can be classified into hepatic vein dominant type（58.75%）,portal vein dominant type（37.50%）,and hepatic vein-portal vein mixed type（3.75%）.Conclusion：Color Doppler ultrasound can noninvasively and accurately evaluate the distribution types of the blood vessels around the gallbladder bed.There were vessel branches not only from hepatic veins but also from portal veins and hepatic arteries in the liver parenchyma around the gallbladder bed.Demonstration of the intrahepatic vessel branches adjacent to gallbladder wall was significant for laparoscopic cholecystectomy.
