益肾运脾汤联合易善复胶囊治疗非酒精性脂肪肝临床研究

摘　　要：目的:探讨益肾运脾汤联合易善复胶囊(多烯磷脂酰胆碱胶囊)对非酒精性脂肪肝(NAFLD)的临床疗效。方法:采用随机、双盲、安慰剂平行对照的研究方法。90例NAFLD患者随机分为中药组、西药组、中西药结合组各30例,中药组采用自拟益肾运脾胶囊及安慰剂胶囊治疗;西药组应用易善复胶囊及安慰剂胶囊治疗,中西药结合组采用益肾运脾胶囊联合易善复胶囊治疗。3月为1疗程。采用症状评分表评价患者疗效,应用彩超检查患者脂肪肝改善程度,并了解肝功能、血脂、脂联素、瘦素、胰岛素抵抗指数(HOMA-IR)等指标变化。结果:中西药结合组在改善症状、改善肝功能、降低血脂、瘦素及胰岛素抵抗水平、提高脂联素水平等方面优于单纯中药组及西药组,差异有统计学意义(P<0.05);中药组在改善症状及降低胰岛素抵抗水平方面优于西药组。结论:益肾运脾汤联合易善复胶囊对非酒精性脂肪肝有更好治疗效果,优于单纯中药及西药疗法;单纯中药疗法优于西药疗法。Objective：To study the therapeutic effect of nourishing the kidney and activating the spleen method combined with polyene phosphatidycholine capsule in the treatment of non-alcoholic fatty liver disease（NAFLD）.Methods：A randomized,double-blind,placebo-controlled and parallel research method was adopted.90 cases of NAFLD patients were randomly divided into three groups of Chinese medicine group,western medicine group and integrative Chinese and western medicine group with 30 patients in each group.The Chinese medicine group was given self-formulated Yishen Yunpi Capsule with polyene phosphatidylcholine capsule as placebo;Western medicine group was given polyene phosphatidylcholine capsule with Yishen Yunpi Capsule as placebo treatment.The integrative Chinese and western medicine group was given Yishen Yunpi Capsule and polyene phosphatidylcholine capsule.Three months are 1 course of treatment.Symptom score sheet was used for the evaluation of effects,the application of ultrasonography was used to examine the improvements of patients with fatty liver,and index changes of liver function,lipids,adiponectin,leptin,insulin resistance index（HOMA-IR） were observed.Results：The integrative Chinese and western medicine group is superior to a single Chinese medicine group and western medicine group in improving symptoms,liver function,lowering blood lipids,leptin and insulin resistance levels and improving the level of adiponectin and the difference was statistically significant（P〈0.05）;Chinese medicine group is superior to western medicine group in improving symptoms reducing the level of insulin resistance.Conclusion：The combination of nourishing the kidney and activating the spleen method with polyene phosphatidycholine capsule can achieve better therapeutic effects in the treatment of NAFLD,which is superior to the single Chinese medicine treatment and western medicine treatment.What’s more,the single Chinese medicine treatment is better than the western medicine treatment.
