摘 要:目的研究延边地区朝鲜族糖尿病肾病(DN)患者CD14启动子-159(C-T)基因的多态性分布,探讨延边地区朝鲜族DN患者该基因的多态性与DN发病的相关情况。方法应用聚合酶链反应-限制性片段长度多态性技术对延边地区人群中152例朝鲜族DN患者及139名朝鲜族正常对照组者的CD14基因启动子-159位点进行基因型分析,并将在该院住院治疗的152例朝鲜族DN患者分为男性和女性两组,分析两者基因型频率和等位基因频率的差别。同时对152例朝鲜族DN患者的临床资料及生化指标进行回顾性分析。结果①CD14启动子-159位点基因多态性在朝鲜族正常对照组和朝鲜族DN患者中的分布差异无显著性(χ2=1.776,P>0.05);②在男性和女性DN的患者中,CD14启动子-159位点纯合子CC的基因频率与杂合子CT及纯合子TT的基因频率相比,差异存在显著性(χ2=3.903,P<0.05);③正常对照组与DN患者相比,DN的发生与高血压、病程、空腹高血糖、C反应蛋白及低密度脂蛋白密切相关(P<0.05或P<0.01),与年龄、餐后血糖、糖化血红蛋白、肌酐、胆固醇、三酰甘油、高密度脂蛋白及体重指数无关(均P>0.05)。结论①CD14启动子-159(C-T)基因的多态性与延边朝鲜族DN患者的发病无关。②CC纯合子基因可能是男性朝鲜族DN患者的易感基因。③高血压、长期病程、空腹高血糖、低密度脂蛋白水平升高及炎症反应是DN发病的重要风险因素。【Objective】 To investigate the distribution of CD14 promoter gene-159(C-T) polymorphism in Korean DN patients of Yanbian area,analyze the association of CD14 polymorphism with Korean DN patients in Yanbian area,and study the risk factors of DN.【Methods】 Analyzed the genotypes of CD14 in 152 Korean DN patients and 139 Korean of normal controls by polymerase chain reaction-restriction fragment length polymorphism,investigated the association between CD14 polymorphism and Korean DN patients in Yanbian area,and conducted a retrospective analysis of clinical features of 152 Korean DN patients.【Results】 ① The distribution of CD14 promoter gene-159(C-T) polymorphism showed no significant difference between normal control group and DN group(χ2=1.776,P 0.05).② The distribution of CC homozygous genotype vs.CT heterozygous and TT homozygous genotype showed statistically significant difference in CD14 C-159T between male and female Korean DN patients(χ2=3.903,P 0.05).③ DN had a close relation with hypertension,process,CRP fasting blood glucose and low density lipoprotein-cholesterol(P 0.05 or P 0.01),but had no relation with Age,PBG,GHb,Cr,CHO,TG,HDL-C and BMI(both P 0.05).【Conclusions】 ① The distribution of CD14 promoter gene-159(C-T) polymorphism has no relation with DN.② The CC genotype of the C-159T polymorphism of CD14 is a susceptibility gene for the male Korean DN patients.③ Hypertension,long process,high fasting blood glucose,low density lipoprotein-cholesterol ascension and inflammatory reaction are important risk factors for DN.
