摘  要:目的比较不同浓度的七氟醚诱导,对新生儿QT间期的影响。方法选择年龄近1个月行择期手术的新生儿60例,体重2.9~4.5 kg。根据呼气末七氟醚浓度的不同随机分为4组:5%组、6%组、7%组和8%组,每组15例。吸入七氟醚4 min,记录患儿入室(T0)、意识消失(T1)及插管后即刻(T2)时点的心率(HR)、血压变化(MAP),同时走纸描记肢体Ⅱ导联下心电图的变化,测量上述时点的QT间期及修正后的QT间期(QTLC)变化。结果与T0相比,同一组的T1和T2时点的HR明显降低(P<0.01),QT、QTLC间期明显延长(P<0.01),T1和T2时点之间的HR、QT、QTLC间期无明显差异;与T 0相比,同一组的T1时点的MAP下降不明显,而T2时点的MAP明显降低(P<0.01);不同组同一时点之间HR、MAP,QT、QTLC差异无统计学意义(P>0.05)。结论七氟醚诱导也可以导致新生儿QT间期延长,但不同七氟醚浓度对新生儿QT间期延长并无明显差别。【Objective】 To compare and assess the inducing effect of sevoflurane with different concentrations on QT interval of newborn.【Methods】 Selected 60 cases of newborns of 1 month old who have undergone elective surgery,weight 2.9~4.5 kg.According to the end-expiratory concentration of sevoflurane the cases were randomly divided into 4 groups: 5% group,6% group,7% group and 8% group,15 cases each.Continuous inhalation of sevoflurane for 4min,heart rate(HR),blood pressure(BP) and electrocardiogram were recorded at the following time points: baseline values(T0),consciousness loss(T1),immediately after intubation(T2).QT interval and QT interval after correction(QTLC) values were measured by electrocardiogram at all three time points mentioned above.【Results】 Compared with T0,HR decreased significantly(P 0.01),QT,QTLC interval were significantly prolonged(P 0.01) at T1 and T2 time points in the same group.HR,QT,QTLC interval had no significant difference between T1 and T2 time points in the same group.Compared with T0,MAP decreased not obviously at T1 time point(P 0.05),but MAP decreased significantly at T2 time point(P 0.01) in the same group.HR,MAP,QT,QTLC interal had no significant difference at the same time point between the four groups(P 0.05).【Conclusion】 Sevoflurane induction can also lead to neonatal QT interval prolongation,but the neonatal QT prolongation has no significant difference between the different end-expiratory sevoflurane concentrations.
