摘 要:目的了解某三级甲等传染病专科医院的医院感染及抗菌药物使用情况。方法对天津市传染病医院2007―2009年各年第四季度某日0∶00-24∶00的全部住院患者进行医院感染现患率调查。结果 2007-2009年该院医院感染现患率分别为4.35%、3.50%、2.17%;最常见医院感染部位依次为上呼吸道、腹盆腔组织、下呼吸道;3年来免疫缺陷、尿管、外周静脉插管和中心静脉插管医院感染危险强度变化差异无统计学意义;共检出9株病原菌,其中革兰阳性菌5株,革兰阴性菌4株;抗菌药物使用以一联用药为主,3年来使用率呈下降趋势(P=0.058);21例次医院感染患者中,16例在调查日使用抗菌药物,且全部为一联用药。结论该院医院感染现患率和抗菌药物使用率均呈下降趋势,医院感染部位以呼吸系统和腹盆腔组织最为常见。OBJECTIVE To study nosocomial infection (NI) and antibiotic usage in a three level of first-class infectious disease hospital. METHODS The point-prevalence survey of NI in all hospitalized patients at one day in the fourth quarter was made from 2007 to 2009. RESULTS The prevalence rates of NI were individually 4.35 %, 3.50% and 2. 17%. The common NI sites were upper respiratory tract, intraabdominal cavity and lower respiratory tract. No statistically difference was found between strength changes of immunodefieiency, urinary catheter, peripheral and central venous catheter. 9 strains of pathogenic bacteria were found through delivering samples including 5 strains of gram-positive bacteria and 4 strains of gram-negative bacteria. Antibiotics utilization rate was decreased in 3 years (P^0. 058), mostly for one couplet medicine. 16 in 21 cases of NI were on antibiotics, and all for one. CONCLUSION The prevalence of NI and antibiotics utilization rate are both decreasing. The most common NI sites are respiratory system and intraabdominal cavity.
