54例肺功能不全的胸部手术患者围术期处理体会

摘　　要：目的探讨部分肺功能不全的开胸手术患者围术期处理方法。方法对2000年1月至2010年1月我科54例合并肺功能不全的开胸患者行充分围术期准备及积极的术后治疗。其中行肺叶切除术16例,肺楔形切除术2例,左进胸食管癌或贲门癌根治术34例,纵隔肿瘤切除术2例。术后均在ICU监护治疗。结果全组无手术死亡病例。出现术后并发症22例,其中呼吸系统并发症17例,死亡1例,余患者均顺利恢复。结论经充分围术期准备及术后得到合理、规范的监护,部分肺功能不全患者仍可进行胸外科手术。Objective To investigate the perioperative treatment for the patients who underwent thoracotomy with respiratory dysfunction.Methods From January 2000 to January 2010,there were 54 patients undergoing thoracotomy with respiratory dysfunction in our center.Some active preoperative and postoperative treatments for them were performed.Eighteen cases had pulmonary lobectomy;6 cases had wedge resection of lung,and 8 cases received wedge resection of lung;34 cases underwent esophagectomy or cardiectomy with left thoracic incision;2 cases had excision of mediastinal tumor.All patients were monitored in ICU.Results There was no dead case during operation.Twenty-two cases suffered from postoperative complications including seventeen cases with respiratory complications and 1 of them has died.The others left hospital successfully without any severe complication.Conclusion Some patients with respiratory dysfunction could undergo a thoracic surgery with adequate treatment before and after operation.
