支架辅助弹簧圈栓塞治疗宽颈动脉瘤的疗效分析

摘　　要：目的探讨支架辅助弹簧圈栓塞治疗宽颈动脉瘤的方法及疗效。方法对18例宽颈动脉瘤应用LEO和SolitaireAB支架辅助AxiumTM机械解脱弹簧圈栓塞治疗,其中11例使用Solitaire AB支架,采用先将支架跨过动脉瘤两端,将微导管置于动脉瘤内距瘤底部1/3-1/2处,弹簧圈部分释放后,再释放支架进行动脉瘤栓塞,解脱支架。7例使用LEO支架,采用放支架、通过支架网孔置入微导管行动脉瘤栓塞。结果 18例患者支架、弹簧圈植入顺利,释放位置满意,技术成功率100%。18枚动脉瘤完全栓塞15枚,占83.33%;大部分栓塞3枚,占16.67%;术后3-6个月随访,未发生出血或脑血栓形成。结论支架辅助弹簧圈栓塞治疗宽颈动脉瘤是一种安全、有效的方法,但应根据动脉瘤的形态、大小、位置以及载瘤动脉的弯曲程度进行支架选择。Objective To evaluate the therpy methods and curative efficacy of technique about disengaging spring coil assisted by stent for embolizing wide neck aneurism.Methods The technique of disengaging AxiumTM coil assisted by LEO and Solitaire AB stent was used in 18 patients.In 11 patients,Solitaire AB stent was planted and strided over the aneurism.Micro catheter was parked inside of aneurism.First,releasing a part of spring coil,then withdraw the stent for going on embolizing aneurism.In 7 patients,LEO stent was deployed,micro catheter was used to embolize aneurism by going through the mesh of stent after that.Results A total of 18 stents were employed successfully.15 patients（83.33%） obtain complete occlusion.3 patients（16.67%） obtain massive occlusion.During the period of 3-6 months after operation,all patients recovered well and the clinical symptoms showed no newly developed bleeding or infarction.Conclusion The technique about disengaging spring coil assisted by stent is safe and effective.Oppropriate stent should be choosed according to the size,form,location of aneurism and the wide degree of artery.
