鼻腔硅胶双气囊通气止血导管在鼻出血治疗中的临床使用和疗效分析

摘　　要：目的比较鼻腔硅胶双气囊通气止血导管和凡士林油纱条在控制鼻腔出血中的疗效。方法 223例成年患者采用鼻腔硅胶双气囊通气止血导管进行治疗(实验组),病例采集时间为2008年1月～2008年12月。对照组为相同时间范围内采取凡士林油纱条进行治疗的患者,共计145例。观察患者疼痛、通气和止血效果。每项指标设定5个级别,对3项主要指标分配不同权重进行综合评价。结果鼻腔硅胶双气囊通气止血导管和凡士林油纱条均有明显的止血效果,两者差异无统计学意义(P>0.05),但在治疗过程中,鼻腔硅胶双气囊通气止血导管在保持鼻腔通气和减轻疼痛方面优于凡士林油纱条(P<0.01)。结论和凡士林油纱条相比,鼻腔硅胶双气囊通气止血导管治疗鼻出血在保证止血效果的同时,更加兼顾了患者鼻腔通气和减轻疼痛等问题,提升了鼻出血患者治疗期间的生活质量。【Objective】 To study the differences between double silicone balloons catheter and the cotton gauze coated with vaseline oil for treatment of epistaxis.【Methods】 223 adult patients with nasal bleeding were adapted into study group and treated by silicone balloons catheter.Other 145 patients treated by cotton gauze coated with vaseline oil were assigned into control group.All cases were collected from Jan to Dec,2008.Hemostatic effect,nasal ventilation and sensation of pain were observed.Five levels were set and scored for each of above objects.These objects were assigned with different weight（0.5,0.25,0.25 respectively） for comprehensive analysis.Meanwhile,we also explore the mucosa trauma variations inside the nasal cavity responding to different materials.【Results】 No significant differences was found in hemostatic effect between two different materials（P 〈0.05）.As to ventilation and pain,double silicone balloons catheter is better than cotton gauze coated with vaseline oil（P〈 0.01）.【Conclusion】 Ventilation and pain were more concerned by using double silicone balloons catheter in the treatment of nasal bleeding.Application of double silicone balloons catheter could elevate the quility of life（QoL） significantly in the process of treatment.
