阿托伐他汀对华法林抗凝作用的影响

摘　　要：目的观察阿托伐他汀对华法林抗凝作用的影响。方法 26例华法林抗凝治疗后国际标准化比值(international normalized ratio,INR)稳定在1.5～2.5之间的患者,每晚加服阿托伐他汀20 mg,观察两药合用前,合用后1、2、3、4周的凝血酶原时间(prothrombin time,PT)I、NR、华法林剂量变化以及出血情况。结果两药合用前后患者的PT及INR差异无统计学意义(P>0.05),合用后华法林每周剂量与合用前平均剂量差异无统计学意义(P>0.05)。26例患者合用药1周后,有2例分别出现轻度齿龈出血和鼻衄,华法林剂量减少0.625 mg后出血均停止。结论阿托伐他汀不影响华法林抗凝作用,不增加出血的风险性,两药合用后也不需要调整华法林剂量。Objective To observe the effect of atorvastatin on the anticoagulation of warfarin. Methods Twenty-six patients with long-term warfarin therapy,whose prothrombin time/international normalized ratio（PT/INR） was maintained between 1.5-2.5,were administered 20 mg atorvastatin once every evening.Changes in（PT/INR and the dosage of warfarin were observed both before and after 1,2,3,4 weeks of combined treatment with the 2 drugs.The occurrence of bleeding was also observed in these 26 patients. Results There were no statistically significant differences in PT and INR both before and after combined administration of the 2drugs（P〉0.05）.No significant differences could be noted in the weekly dosage and average dosage of warfarin,following combined administration of the drugs（P〉0.05）.After one week of combined administration of the 2drugs,slight gingival bleeding and nose bleeding occurred in 2 cases of the 26 patients.However,bleeding was stopped,following a reduced dosage of warfarin by 0.625 mg. Conclusion Atorvastatin seems to have no effect on the anticoagulation of warfarin.The risk of hemorrhage seems to be very low,following combined treatment.For this reason,there is no need for the adjustment of the dosage of warfarin.
