摘  要:目的探讨足月妊娠羊水偏少孕妇运用生物物理评分监测下阴式分娩的安全性。方法 176例经超声诊断羊水偏少的孕妇(观察组)与同期213例羊水正常的孕妇(对照组),通过产前监测胎儿生物物理评分、催产素激惹试验,临产后持续胎心监护,比较2组分娩方式、胎儿结局及对新生儿发病率的影响。结果观察组变异减速发生率高于对照组(P<0.05);2组阴道分娩、剖宫产、胎儿窘迫和新生儿窒息发生率比较差异无统计学意义(P>0.05),观察组中社会因素为剖宫产指征者明显高于对照组(P<0.05)。结论应用胎儿生物物理评分监测产程中胎儿状况,可有效避免医源性手术,降低母婴风险。Objective To explore the safety of labor trial in full-term oligohydramnios pregnant women under the monitoring with fetal biology-physics evaluation.Methods Oxytocin challenge test in prepartum and fetal heart rate testing were used in antepartum to analyze the delivery pattern,maternal and fetal outcome,and the morbidity of neonatal diseases in 176 oligohydramnios pregnant women diagnosed by ultrasound(observation group) and 213 normal cases in the same period(control group).Results The variable deceleration rate and cesarean section rate without medical indications were higher in the observation group than those in control group(P0.05).There were no significant differences in the incidence of vaginal delivery rate,cesarean section rate,fetal distress or neonatal asphyxia between two groups(P0.05).Conclusion Monitoring the fetus condition by fetal biology-physics evaluation can avoid the unnecessary obstetric intervention in full-term pregnant women with oligohydramnios.
