后腹腔镜技术治疗肾盂源性囊肿6例临床分析

摘　　要：目的总结后腹腔镜治疗肾盂源性囊肿的经验。方法 6例患者均为术中诊断为肾盂源性囊肿,4例术前予输尿管逆行插管留置输尿管导管。均采用三孔后腹腔镜肾囊肿去顶术治疗,术中关闭囊肿与肾盂间的通道;术后置入患侧肾盂输尿管双J管。结果 4例于术中经输尿管导管注射亚甲蓝,发现集合系统与囊肿有通道存在而确诊;2例术中明确见集合系统与囊肿有通道后确诊。6例患者均顺利完成手术,术后第4 d拔除腹膜后引流管,无漏尿发生,切口均Ⅰ期愈合。术后1个月经膀胱镜拔除双J管。随访3～6个月,复查B超囊肿无复发。结论后腹腔镜技术治疗肾盂源性囊肿效果良好,术前输尿管逆行插管可提高其诊断率。Objective To summarize the experiences of treating pyelogenic cyst with retroperitoneal laparoscopy.Methods 6 patients were diagnosed pyelogenic cyst during operation.4 patients were given retrograde Ureteral Catheterization and indwelled ureteral catheter before operation.All patients were given cyst unroofing.Channel between cyst and pelvis were closed during operation,and double J pelvis-ureteral stent was given in suffered side after operation.Results The operation was successfully done in all patients.Retroperitoneal Indwelling Drain was pulled out at the fourth day after operation,and no patients had leakage of urine.All incisions were primary Healing.Double J stent was pulled out through cystoscope after one month of operation.A follow-up of 3 to 6 months showed no recurrence in all patient.Conclusion Technique of retroperitoneal laparoscopy in the treatment of pyelogenic cyst was characterized by good efficacy.
