自制体位架拍摄腰椎功能位平片在诊断腰椎滑脱中的价值

摘　　要：目的探讨应用自制体位架拍摄腰椎功能位X线平片,在诊断腰椎滑脱中的价值。方法对94例腰腿痛和坐骨神经痛的患者,采用腰椎体位架下功能位和常规侧卧位X线平片两种方法,测量和诊断有无腰椎滑脱及其程度,进行统计学分析与比较。结果本组94例显示腰椎滑脱共计55例,常规侧卧位阳性39例,功能位阳性55例,其中过伸位51例,过曲位45例;腰椎滑脱程度功能位较侧卧位有显著的加重趋向,差异有统计学意义(P=0.000)。结论自制腰椎体位架的功能位X线平片,更能反映腰椎滑脱的实际程度。Objective To investigate the value of X-ray film of lumbar functional position in the diagnosis of lumbar spondylolisthesis by the self-made position frame.Methods Using two methods of X-ray including lumbar functional postion by the frame and lying lateral position respectively to detect and diagnose the lumbar spondylolisthesis and its extent of 94 cases with lumbago,scelalgia and sciatica,then to analyze and compare the datas statistically.Results It shows a total of 55 cases of lumbar spondylolisthesis among 94 cases,and there are 39 cases positive by the conventional lateral positions,55 cases positive by the functional position,including 51 cases by hyperextention and 45 cases by over-bending position.Compared with the degree of spondylolisthesis diagnosed by the generally lateral position,that diagnosed by lumbar functional position tends to increase significantly,the difference between them has statistically significance（P=0.000）.Conclusion The X-ray film of functional position by self-made positon frame can diagnose the actual degree of spondylolisthesis more accurately than the generally lying lateral position.
