子宫动脉化疗栓塞治疗剖宫产切口妊娠的临床价值

摘　　要：目的探讨经子宫动脉化疗栓塞对剖宫产切口妊娠(CSP)的治疗价值。方法回顾性分析2006年6月～2010年6月经子宫动脉化疗栓塞治疗的16例剖宫产切口妊娠病例。其中12例为术前B超诊断为切口妊娠;3例为人流术中出血多或术后反复大量出血;1例为药流后反复大量阴道出血。14例栓塞治疗后1周内在B超监测下行清宫术,观察术中出血量。结果所有患者均成功施行双侧子宫动脉化疗栓塞术。14例在1周内行B超监察下清宫术,术中出血少,无一例子宫切除。结论子宫动脉化疗栓塞治疗剖宫产切口妊娠,不仅可以有效杀死妊娠物,减少清宫出血量,为剖宫产后切口妊娠的保守治疗提供安全保障,而且对切口妊娠清宫术中大出血的患者可以达到即时止血的目的。是治疗剖宫产切口妊娠安全、有效的方法。[ Objective ] To probe the clinieal value of uterine artery chemoembolization in the treatment of caesarean scar pregnancy （CSP）. [Methods ] Retrospectively analyze the 16 cases of the caesarean scar pregnancy during the period of June 2006 to June 2010 that under the treatment of uterine artery ehemoembolization, of which, 12 are diagnosed by type-B ultrasonic before operation, 3 in the treatment of the abortion with massive haemorrhage or repeatedly haemorrhage, 1 in the medical abortion with repeatedly massive vaginal haemorrhage. 14 cases took the operation of curettage under the monitoring of type-B Ultrasonic one week after the uterine artery ehemoembolization, and observed the bleeding output in the operation; [ Result ] All the patients successfully took the operation of dual uterine artery chemoembolization, 14 took the operation of curettage under the type-B ultrasonic monitoring in a week, small sum of bleeding and no uterectomy; [ Conclusion ] Uterine artery chemoembolization is a safe and effective way to treat the caesarean sear pregnancy （CSP）. It not only effectively kills the trophoblastic tissue, reduces bleeding in curettage, but also provides safeguard for the conservative treatment of the caesarean scar pregnancy （CSP）, beats the target of instant hemostasis in the process of curettage of caesarean scar pregnancy （CSP）.
