Rigidfix系统固定重建前交叉韧带的疗效比较

摘　　要：目的探讨关节镜下应用Rigidfix系统及挤压螺钉固定重建前交叉韧带(ACL)的临床效果及自体与同种异体肌腱重建的疗效比较。方法 2005年7月～2008年5月对该院共42例关节镜下确诊的ACL断裂的患者行双骨道单束自体肌腱或同种异体肌腱韧带重建治疗,应用Rigidfix系统及挤压螺钉对重建韧带进行固定,术后完成相关随访,随访主要包括关节活动度,膝关节查体,MRI影象等,功能评定采用Lysholm评分系统。结果所有患者随访1.5～4.0年,平均2.8年。其中1例出现术后感染的患者膝关节屈曲受限外,余患者末次随访所有膝关节伸-3°～0°,屈110°～130°,2例患者前抽屉试验、Lachman试验为阳性,Lysholm评分由术前(34.6±4.36)分提高至术后(89.4±5.42)分。采用自体肌腱重建患者Lysholm评分由术前(32.4±6.95)分提高至术后(91.3±5.51)分,同种异体肌腱重建患者由术前(35.6±4.49)分提高至术后(90.2±6.32)分,两组患者功能比较差异无显著性(P>0.05)。结论关节镜下应用Rigidfix系统及挤压螺钉固定自体及同种异体肌腱重建前交叉韧带均可获得较为满意的临床疗效。[Objective] To evaluate the clinical effects of fixation for anterior cruciate ligament reconstructed with Rigidfix system and interference screw fixation and to compare the effects of autologous and allogeneic tendon grafts.[Methods] From July 2005 to May 2008,42 cases with ACL rupture diagnosed by arthroscopy had been reconstructed with allogeneic or autologous tendon.Then all the tendons were fixed by Rigidfix system and interference screw.Rehabilitation exercise was taken after the operation.The patients were followed up for ROM,objective sign and MRI images.Lysholm scoring system was used to evaluate functions of the knees.[Results] The follow-up averaged time was 2.8（1.5-4） years.Except one case with infection gotten flexion limitation,all the others had no severe complications such as infection,immunological rejection and so on.At the final follow-up,the motion of the treated knees of the others ranged from-3° to 0° in extension and from 115° to 130° in flexion.For two cases,the results of anterior drawer test（ADT）,Lachman test and pivot shift test were negative.The Lysholm score changed from（34.6±4.36） in preopteration to（89.4±5.42） in postopteration.[Conclusion] Fixation of anterior cruciate ligament with Rigidfix system and interference screw for anterior cruciate ligament which have been reconstructed by allogeneic and autologous tendons has an satisfactory clinical therapeutic effect.
