垂体后叶素在腹腔镜输卵管妊娠切开术中的应用及对术后生殖功能的影响

摘　　要：目的探讨垂体后叶素在腹腔镜输卵管妊娠切开术中的应用及对术后生殖功能的影响。方法回顾性分析2006年6月～2009年3月该院行腹腔镜下输卵管切开术358例患者的临床资料,其中观察组235例应用垂体后叶素,对照组123例未用垂体后叶素,比较两组手术时间、术中出血量、输卵管切除率、术后最高体温、持续性异位妊娠发生率以及术后患侧输卵管通畅率、宫内妊娠率、重复异位妊娠率和不孕率。结果观察组的手术时间、术中出血量、输卵管切除率、术后持续性异位妊娠发生率明显少于对照组,两组比较差异有显著性(P<0.05);两组患者术后生殖结局比较,观察组较对照组有更高的患侧输卵管通畅率、宫内妊娠率和较低的不孕率,两组比较差异有显著性(P<0.05),两组重复异位妊娠率比较差异无显著性(P>0.05)。结论腹腔镜输卵管妊娠切开术中应用垂体后叶素可以缩短手术时间,减少术中出血,降低持续性异位妊娠的发生,保护输卵管卵巢的功能。[Objective] To explore the use of hypophysin in the salpingotomy by laparoscope and its influence on reproduction function after the operation.[Methods] Retrospectively analyzed 358 cases of salpingotomy by laparoscope in our hospital during 2006-6 to 2009-3,among which,the total 235 cases classified into experiment group were utilized hypophysin,and 123 cases classified into control group were not utilized hypophysin.Compared the two groups in operation time,hemorrhage volume in operation,the rate of tubal resection,the highest body temperature after operation,the incidence rate of durative eccyesis,the patency rate of gravidity site oviduct after operation,the rate of intrauterine gestation after operation,the rate of repeat eccyesis after operation and the rate of barrenness after operation.[Results] The operation time,hemorrhage volume in operation,the rate of tubal resection and the incidence rate of durative eccyesis in experiment group were lower than those in the control group obviously.The differences between the two groups were significant（P 0.05,P 0.01）.Contrasting the patients in the two groups in reproduction results,the experiment group had a higher rate in the smooth of gravidity site oviduct and intrauterine gestation after operation and a lower rate of barrenness after operation comparing to control group.The difference in the two groups had a significance（P 0.05）.The difference of the rate of repeat eccyesis after operation in the two groups had no significance（P 0.05）.[Conclusions] The use of hypophysin in the salpingotomy by laparoscope can shorten operation time,cut down the incidence of durative eccyesis,safeguard the function of oviduct and ovaries and raise the rate of intrauterine gestation.
