腹腔镜手术治疗腔静脉后输尿管

摘　　要：目的探讨腹腔镜和传统开放手术治疗腔静脉后输尿管的疗效。方法回顾分析该院1988年～2010年26例腔静脉后输尿管患者的临床资料。其中15例采用传统开放手术治疗,11例采用腹腔镜手术治疗。比较两种方法在手术时间、出血量、术后肠功能恢复时间、住院时间及并发症情况。结果腹腔镜手术组手术时间(72.6±21.7)min,出血量(54.1±16.7)mL,胃肠功能恢复时间(2.2±0.5)d,住院天数(7.2±1.9)d,没有出现并发症。开放手术组手术时间(136.8±19.8)min,出血量(245.3±112.2)mL,胃肠功能恢复时间(2.4±0.6)d;住院天数(11.7±4.8)d,3例发生切口感染或脂肪液化。两组手术时间、出血量和住院时间比较,差异具有显著性(P<0.05)。结论腹腔镜是目前治疗腔静脉后输尿管最佳手术方式。[Objective] To study the treatment effects of laparoscopic and opening operation on circumcaval ureter.[Methods] Retrospective1y analyzed the data of 26 patients with circumcaval ureter（16 patients with opening surgery,11 patents with laparoscopic surgery） and compared the outcomes in operative time,bleeding volume,postoperative intestinal function recovery time,hospital stay and complications.[Results] In the laparoscopic surgery group,operative time was（72.6±21.7） min,bleeding volume was（54.1±16.7） mL,postoperative intestinal function recovery time was（2.2±0.5） d,hospital stay was（7.2±1.9） d,and no complications occurred.In the opening surgery group,operative time was（136.8±19.8） min,bleeding volume was（245.3±112.2） mL,postoperative intestinal function recovery time was（2.4±0.6） d,hospital stay was（11.7±4.8） d,and postoperative infection occurred in 3 cases.The difference on operative time,bleeding volume,hospitalization were significant（P 0.05）.[Conclusions] Laparoscopic surgery is the best operation for patients with circumcaval ureter at present.
