卡泊三醇联合复方丙酸氯倍他索软膏治疗银屑病的近期疗效观察

摘　　要：目的探讨卡泊三醇软膏联合复方丙酸氯倍他索软膏外用治疗银屑病的近期临床疗效。方法采用随机、开放、平行自身对照试验。将符合入选标准的93例慢性斑块型银屑病患者的皮损沿躯干中线分为左右两侧,采用随机数字法决定一侧躯干及肢体皮损为治疗组,给予外用复方卡泊三醇软膏,2次/d,复方丙酸氯倍他索软膏,1次/d,4周为1个疗程,与治疗部位相对应的另一侧躯干及肢体皮损为对照组,涂用丙酸氯倍他索软膏,2次/d,他扎罗汀乳膏,1次/d,疗程同治疗组。同时给予患者口服维生素C 0.2,3次/d,叶酸10 mg,3次/d,郁金银屑片0.96,3次/d,窄谱中波紫外线治疗1次/3 d。治疗前及治疗开始后每2周评估患者的症状积分,计算患者的症状积分指数(SSRI),评价治疗效果。结果共有79例患者完成试验,治疗组D14、D28时SSRI分别为0.81±0.23与0.92±0.11,与对照组(0.53±0.17、0.64±0.23)相比差异均有统计学意义(均P<0.05);治疗组痊愈率D14为63.29%,D28为78.48%,与对照组(分别为39.24%、50.63%)相比差异有统计学意义(均P<0.05);总有效率治疗组在D14为94.94%,D28为97.47%,与对照组相比较(D14为65.82%,D28为72.15%)差异均有统计学意义(均P<0.05)。结论卡泊三醇软膏联合复方丙酸氯倍他索软膏外用治疗寻常型银屑病具有肯定的疗效,未发现严重不良反应。Objective To observe short-term outcomes of calcipotriol ointment combined compound clobetasol propionate ointment in psoriasis treatment. Methods A randomized, parallel, positive medicine serf-controlled trial was administered in 93 subjects with chronic plaque psoriasis. Psoriatic lesions in every patients were divided to two parts according body surface medine line, randomized number were used to choose lesions on left or right body and limbs as treatment group which received external use of calcipotriol ointment, twice a day, compound clobetasol propionate ointment, once a day. 4 weeks as a period of treatment. Meanwhile, the other parts of body and limb were choose as control group which received external clobetasol propionate cream, twice a day, tazarotine cream, once a day. 4 weeks as a period of treatment. Symptom score reducing index（SSRI） were evaluated in each group before and every 2 weeks after treatment beginning. Results 79 subjects were completed the trial. SSRI in treatment group on D14, D28 were 0.81 ± 0.23 and 0.92 ± 0.11, respectively, which were statistically higher than control group （0.53 ±0.17 and 0.64 ± 0.23, both P 〈 0.05 ）. Cure rate in treatment group was 63.29% on D14 and 78.48% on D28, which were both markedly increased compared to control group （39.24% on D14 and 50.63% on D28, both P 〈 0.05 ）. Total effective rate in treatment group was 94.94% on D14 and 97.47% on D28, which were both significantly higher compared with control group （65.82% on D14 and 72.15% on D28, both P 〈 0.05 ）. Conclusion These results suggest that external use of calcipotriol ointment combined compound clobetasol propionate ointment in treatment of psoriasis vulgaris is effective without serious adverse reaction. It＇s worthy of clinic application.
