白细胞介素-6与慢性心衰的相关性及血管紧张素转换酶抑制剂的影响

摘　　要：目的:探讨慢性心力衰竭(CHF)患者血清白细胞介素-6(IL-6)水平的变化及血管紧张素转换酶抑制剂(ACEI)的影响。方法:采用双抗体夹心酶联免疫吸附法(ELISA)检测38例CHF患者及23例健康自愿者(健康对照组)血清IL-6水平,CHF患者被随机分为常规治疗组(17例)和ACEI治疗组(21例,常规治疗+ACEI治疗)共观察4周。结果:①CHF患者血清IL-6水平显著高于健康对照组[(9.82±4.67)ng/L比(4.2±2.65)ng/L,P<0.01],且与左室射血分数(LVEF)呈负相关(r=-0.55,P<0.01);②经4周治疗后与治疗前比较,ACEI组患者IL-6水平明显降低[(9.86±4.57)ng/L比(7.43±4.39)ng/L,P<0.05],且伴有心率明显下降[(84±9)次/min比(76±8)次/min,P<0.05],LVEF明显升高[(32±3.2)%比(37±4.7)%,P<0.05],常规治疗组治疗前后无显著差异(P>0.05)。结论:慢性心力衰竭患者白细胞介素-6水平明显升高,并与左室收缩功能呈负相关,血管紧张素转换酶抑制剂可降低血清白细胞介素-6水平,有助于慢性心力衰竭患者心功能的改善。Objective： To study correlation between serum level of interleukin--6 （IL--6） and chronic heart failure （CHF） and effects of angiotensin converting enzyme inhibitors （ACEI） on CHF. Methods： Enzyme linked immu- nosorbent assay （ELISA） was used to measure serum level of IL- 6 in 38 CHF patients and 23 healthy subjects （healthy control group）. CHF patients were randomly divided into routine treatment group （n= 17） and ACEI treatment group （n=21, received routine ~ ACEI treatment） and they were observed for four weeks. Results： （1） Serum level of IL--6 in CHF patients was significantly higher than that of healthy control group [ （9.82±4.67） ng/ L vs. （4.2±2.65） ng/L, P〈0.01], and it was negatively related with left ventricular ejection fraction （LVEF, r =--0.55, P〈0.01）; （2） After four-week treatment, compared with before treatment, there were significant decrease in IL--6 level [（9.86±4.57） ng/L vs. （7.43±4.39） ng/L, P〈0. 051 and heart rate [ （84±9） times/rain vs. （76±8） times/min, P%0.051, and significant increase in LVEFE （32±3.2）% vs. （37±4.7）%, P〈0.05], but there was no significant changes in routine treatment group before and after treatment （P〉0.05）. Conclusion：Serum level of IL-6 significantly increases in CHF patients and it＇s negatively correlated with left ventricular systolic function. ACEI can lower serum level of IL--6 and is helpful to improve cardiac function in CHF patients.
